不同手术方法治疗异位妊娠的效果比较(1)
[摘要]目的 比較不同手术方法治疗异位妊娠的临床效果。方法 选取2017年6月～2018年6月我院收治的60例异位妊娠接受输卵管切除手术产妇作为研究对象，根据治疗方法的不同分为单侧组和单+对侧组，每组各30例。单侧组实施单侧输卵管切除手术，单+对侧组实施单侧输卵管切除+对侧输卵管结扎手术。比较两组产妇术前与术后1、2、3个月的抗苗勒管激素(AMH)水平和Kupperman评分;采用Pearson检验分析AMH水平和Kupperman评分的相关性。结果 术前，两组的AMH水平和Kupperman评分比较，差异无统计学意义(P>0.05);两组术后1、2、3个月的AMH水平低于术前，Kupperman评分高于术前，差异均有统计学意义(P<0.05);单侧组术后1、2、3个月的AMH水平高于单+对侧组，Kupperman评分低于单+对侧组，差异均有统计学意义(P<0.05)。术前、术后1、2、3个月的AMH和Kupperman评分均成显著负相关(r=-0.593，P=0.041;r=-0.758，P=0.004;r=-0.674，P=0.016;r=-0.603，P=0.038)。结论 接受输卵管切除术可能影响异位妊娠患者的卵巢功能，导致AMH水平降低，且实施单侧输卵管切除手术能缓解AMH水平的下降程度和卵巢功能损伤。

[Abstract] Objective To compare the clinical effects of different surgical methods in the treatment of ectopic pregnancy. Methods A total of 60 parturients with ectopic pregnancy who underwent salpingectomy in our hospital from June 2017 to June 2018 were selected as the research objects， they were divided into unilateral group and unilateral+contralateral group according to the different treatment methods， 30 cases in each group. Unilateral salpingectomy was performed in the unilateral group， while unilateral salpingectomy and contralateral tubal ligation were performed in the unilateral+contralateral group. The levels of anti mullerian hormone (AMH) and Kupperman score in preoperative and 1， 2 and 3 months after operation were compared between the two groups. Pearson test was used to analyze the correlation between AMH level and Kupperman score. Results There were no significant differencs in AMH level and Kupperman score between the two groups before operation (P>0.05). The AMH level of the two groups at 1， 2 and 3 months after operation was lower than that before operation， and the Kupperman score was higher than that before operation，the differences were statistically significant (P<0.05). AMH level in unilateral group was higher than that in unilateral + contralateral group at 1， 2 and 3 months after operation， and Kupperman score was lower than that in unilateral+contralateral group (P<0.05). AMH and Kupperman scores were negatively correlated before and 1， 2 and 3 months after operation (r=-0.593， P=0.041; r=-0.758， P=0.004; r=-0.674， P=0.016; r=-0.603， P=0.038).
