骨性Ⅱ类错不同垂直骨面型的切牙代偿研究(1)

     [摘要]目的 探討分析成人骨性Ⅱ类成人错 不同垂直骨面型患者切牙代偿情况的差异,为临床诊治提供参考依据。方法 选取2015年1月~2018年1月我院收治的105例成人骨性Ⅱ类错 患者作为研究对象,所有患者治疗前进行头颅侧位片拍摄,并按颅底平面和下颌平面夹角(SN/MP角)将患者分为高角组(n=23)、均角组(n=47)、低角组(n=35)三种不同的垂直骨面型,将成人骨性Ⅱ类错 患者与正常 的头影测量结果比较,并分析各组切牙倾斜度指标的差异。结果 成人骨性Ⅱ类错 患者的下颌中切牙与下颌平面交角(L1/MP)、下颌中切牙与NB平面交角(L1/NB)高于正常 ,上下颌的中切牙夹角(U1/L1)则低于正常 ,差异有统计学意义(P<0.05);成人骨性Ⅱ类错 患者的其他上切牙唇舌向倾斜程度指标与正常 比较,差异无统计学意义(P>0.05)。在上切牙唇倾度方面,高角组的上颌中切牙与颅底平面夹角(U1/SN)低于低角组,上颌中切牙与上颌平面交角(U1/PP)高于低角组,差异有统计学意义(P<0.05);在下切牙唇倾度方面,低角组的L1/MP、下颌中切牙与NB平面交角(L1/NB)高于高角组,差异有统计学意义(P<0.05)。结论 成人骨性Ⅱ类错 患者的上切牙无明显的倾斜代偿,但下切牙倾斜代偿明显。不同骨面型患者上下切牙的唇舌向代偿有所差异,在临床矫治时应结合患者矢状面不调程度和不同垂直面骨型的切牙代偿情况进行综合分析,以获得更好的矫治效果。

    [Abstract] Objective To explore and analyze the difference of incisor compensation in adult class Ⅱ malocclusion patients with different vertical bone facial types, and to provide reference for clinical diagnosis and treatment. Methods From January 2015 to January 2018, 105 adult patients with skeletal class Ⅱ malocclusion in our hospital were selected as the research objects, all patients received lateral cephalometry before treatment. According to the angle between the skull base plane and the mandibular plane (SN/MP angle), the patients were divided into the high angle group (n=23), the mean angle group (n=47) and the low angle group (n=35). The cephalometric results of adult class Ⅱ malocclusion were compared with those of normal occlusion, and the difference of incisor inclination in each group was analyzed. Results The angle of intersection between mandibular central incisor and mandibular plane (L1/MP), angle between mandibular central incisor and NB plane (L1/NB) of adult patients with class Ⅱ malocclusion was higher than that of normal occlusion, and the angle between upper and lower central incisors (U1/L1) was lower than that of normal occlusion (P<0.05). There was no significant difference in the degree of lip and tongue inclination between other incisors of adult patients with class Ⅱ malocclusion and normal occlusal teeth (P>0.05). In the upper incisor, the angle between maxillary central incisor and skull base plane (U1/SN), angle between maxillary central incisor in the high angle group was lower than that in the low angle group, maxillary plane (U1/PP) in the high angle group was lower than that in the low angle group, angle between maxillary central incisor in the high angle group was higher than that in the low angle group, and the differences were difference was statistically significant (P<0.05). In the lower incisor labial inclination, the angle of intersection between mandibular central incisor and mandibular plane (L1/MP), angle between mandibular central incisor and NB plane (L1/NB) in the low angle group was higher than that in the high angle group, and the difference was difference was statistically significant (P<0.05). Conclusion The superior incisor of adult patients with class Ⅱ malocclusion had no obvious oblique compensation, but the inferior incisor was significantly compensated. However, there are differences in the lip and tongue compensation of the upper and lower incisors of different bone facial types. In order to obtain better curative effect, the patients′ sagittal unadjusted degree and the different vertical facial types′ compensation should be combined in clinical treatment., 百拇医药(刘聪 肖振朋 黄坚愧)
