姑息照护对淋巴瘤患者生命质量的影响(1)
[摘要]目的 探讨姑息照护对淋巴瘤患者生命质量的影响。方法 选取2015年12月~2018年2月我院收治的100例淋巴瘤患者,按照随机数字表法分为研究组和对照组,每组各50例。对照组给予常规基础护理、专科护理、心理护理、健康教育。研究组在常规护理基础上给予姑息照护,包括症状管理、信息需求管理、心理支持和人文关怀。比较两组干预前、干预后12周的生命质量评分、症状评分、焦虑抑郁评分。结果 研究组干预后的生命质量各维度评分、生命质量总分均明显高于干预前、对照组干预后,差异有统计学意义(P<0.01)。研究组干预后的焦虑、抑郁评分均明显低于干预前、对照组干预后,差异有统计学意义(P<0.01)。研究组干预后的各症状评分均明显低于干预前、对照组干预后,差异有统计学意义(P<0.01)。结论 姑息照护能够有效提高淋巴瘤患者的生命质量,改善心理健康状态,缓解临床症状。

[Abstract] Objective To explore the impact of palliative care on the quality of life of patients with lymphoma. Methods The 100 cases of lymphoma patients admitted to our hospital from December 2015 to February 2018 were selected and divided into the study group and the control group according to the random number table method, 50 cases in each group. Routine basic nursing, specialized nursing, psychological nursing and health education were given to the control group during hospitalization. The study group was provided palliative care on the basis of conventional nursing, including symptom management, information needs management, psychological support and humanistic care. Life quality score, symptom score, anxiety and depression score were compared between the two groups before and after intervention 12 weeks. Results The scores of all dimensions and total scores of quality of life in the study group after intervention were significantly higher than those before intervention and after intervention in the control group, with statistical differences (P<0.01). The scores of anxiety and depression in the study group after intervention were significantly lower than those before intervention and after intervention in the control group, and the differences were statistically significant (P<0.01). The scores of symptoms in the study group after intervention were significantly lower than those before intervention and after intervention in the control group, and the differences were statistically significant (P<0.01). Conclusion Palliative care can effectively improve the quality of life of patients with lymphoma, improve mental health and alleviate clinical symptoms.

淋巴瘤臨床常用的治疗方法包括化疗、放疗、生物治疗、分子靶向治疗、生物免疫治疗等,患者不仅要承受症状带来的痛苦,常常还伴随严重的情感压力[1-2]。姑息照护是为患有威胁生命的疾病的患者和家属提供的一种照护方法,目标是尽力帮助终末期患者和家属获得最好的生存质量。以镇痛、控制其他症状,减轻精神心理创伤,帮助患者解决生存期的某些社会问题。2010年,美国麻省总医院肿瘤中心发表于《新英格兰医学杂志》的关于姑息治疗可延长进展期非小细胞肺癌患者生存期的临床研究报告受到普遍关注,提高了肿瘤工作者对姑息医学实践的参与热情,使业内有识之士从全新的角度对姑息医学的内涵进行了审视[3]。本研究将姑息照护应用于临床淋巴瘤患者的管理中,探讨其对患者生命质量的影响。, 百拇医药(成瑜 冯智超 曾纯 马淑玲)
