MR扩散峰度成像技术在轻度认知功能障碍患者额叶前皮质微结构变化中的应用价值
[摘要]目的 分析MR擴散峰度成像(DKI)技术在轻度认知功能障碍(MCI)患者额叶前皮质微结构变化中的应用价值。方法 回顾性分析2018年5月~2019年4月我院收治的93例脑小血管病患者的临床资料,其中未伴有MCI的45例患者作为对照组,存在MCI的48例患者作为观察组。两组患者均接受常规MR及DKI扫描。比较两组患者感兴趣区的平均扩散峰度(MK)、轴向扩散峰度(AK)、扩散各向异性分数(FA)值。结果 观察组患者右侧前扣带和旁扣带脑回MK值为(0.66±0.08)mm2/s,AK值为(0.63±0.07)mm2/s,FA值为(0.16±0.04)mm2/s,低于对照组的(0.72±0.05)、(0.69±0.05)、(0.19±0.02)mm2/s,差异均有统计学意义(P<0.05);两组患者左侧前扣带和旁扣带脑回、双侧背外侧额上回MK、AK、FA值比较,差异均无统计学意义(P>0.05)。结论 DKI技术可作为观察MCI患者额叶前皮质微结构变化的有效方法,MK、AK、FA参数可以为临床评估提供重要信息。

[Abstract] Objective To analyze the application value of MR diffusion kurtosis imaging (DKI) in the microstructural changes of the frontal cortex of patients with mild cognitive impairment (MCI). Methods The clinical data of 93 patients with cerebral small vessel disease treated in our hospital from May 2018 to April 2019 were retrospectively analyzed. Among them, 45 patients without MCI were selected as the control group, and 48 patients with MCI were selected as the observation group. Both groups received regular MR and DKI scanning. The mean kurtosis (MK), axial kurtosis (AK), and diffusion fraction anisotropy (FA) values of the two groups in interest area were compared. Results The MK of the right anterior and side cingulate cerebral gyrus of the observation group was (0.66±0.08) mm2/s, AK was (0.63±0.07) mm2/s, and FA was (0.16±0.04) mm2/s, which lower than those in the control group for (0.72±0.05), (0.69±0.05), (0.19±0.02) mm2/s, respectively, and the differences were statistically significant (P<0.05). There were no significant differences in the MK, AK and FA values of the left anterior cingulate and side cingulate cerebral gyrus and the bilateral dorsolateral gyrus between the two groups (P>0.05). Conclusion DKI technology can be used as an effective way to observe the microstructural changes of the frontal cortex in patients with MCI, and the MK, AK, FA parameters can provide important information for clinical evaluation.
