微创稳定系统治疗复杂性股骨干骨折37例临床分析(1)

【摘要】 目的:探讨微创稳定系统(Less Invasive Stabilization System,LISS)治疗复杂性股骨干骨折的临床效果。方法:2008年1月-2011年6月,采用LISS钢板内固定术治疗复杂性股骨干骨折37例,AO分型:B型11例,C型26例;其中开放性骨折6例,合并骨盆骨折1例,合并腰椎骨折2例,合并同侧尺骨骨折1例,合并同侧胫腓骨骨折2例,合并颅脑损伤1例。术后根据膝关节功能Kolment评定标准评价疗效。结果:随访37例,时间8~16个月,平均6个月复查X线片示骨折对位、对线佳,骨折均愈合,无钢板断裂和螺钉松动。优21膝,良12膝,可3膝,差1例,优良率为89.2%。结论:应用LISS钢板内固定技术具有手术切口小、出血少、感染少等优点。而且骨折固定可靠,无需外固定、骨折愈合快,可早期活动,关节功能恢复满意,是治疗复杂性股骨干骨折安全、有效的方法之一,值得在临床上进一步推广。

【Abstract】 Objective:To investigate the clinical effect of less invasive stabilization system ( Less Invasive Stabilization System LISS ) in treatment of complex femoral shaft fractures.Methods:From January 2008 to June 2011, 37 type complex femoral shaft fractures were treated by LISS plate fixation, AO classification:11 of type B, 26 of type C; Including 6 cases of open fracture, 1 case of pelvic fracture, 2 cases of lumbar fracture , 1 case of ipsilateral fracture , 2 cases of lateral tibial and fibular fracture, 1 case of craniocerebral injury. The postoperative knee function evaluation based on Kolment standard for the evaluation of curative effect.Results:37 cases were followed up for 7-16 months, Analysis of reviewing the X-ray film average 6 months revealed that all the fractures were good to the line and position and healed without breakage of steel plate and screw loosening, the score of the function of knee joint according to Kolment standard: excellent in 21 cases, good in 12 cases, ordinary in 3 cases, poor in 1 case, the rate of the excellent and good ones was 89.2%.Conclusion:The application of LISS steel plate internal fixation has the advantage of small incision, less bleeding,less infection ,etc.The effect of fracture fixation is reliable and can do without external fixation,which is benefit for the fracture healing, the exercise at the early stage and the satisfactory functional recovery. LISS is a safe and effective approach in the treatment of complex femoral shaft fracture, which is worth of the further promotion for clinical application.
