小儿肱骨髁上骨折采用闭合复位克氏针固定术的临床效果分析(1)
【摘要】 目的：研究分析采用闭合复位克氏针固定术治疗小儿肱骨髁上骨折的临床效果并作出评价。方法：依据纳入排除标准，选取2012年12月-2014年12月在本院儿科接受手术治疗的120例肱骨髁上骨折的患儿作为临床研究对象，并将患儿按照随机数字表法分为研究组和对照组，每组各60例。对照组采用临床上常用的切开复位克氏针固定治疗方法，研究组采用闭合复位克氏针固定术治疗。在治疗后的6～12个月，对两组患儿治疗后的恢复情况进行回访调查，作出评分比较其临床效果差异。结果：两组患儿经手术治疗后肱骨髁上骨折处均愈合，研究组患儿的手术时间、住院时间及骨折部位愈合时间与对照组比较均明显缩短，差异均有统计学意义(P<0.05)。研究组患儿术后恢复评定等级为优的30例，良29例，一般1例，对照组患儿评定等级为优的14例，良23例，一般21例，组间差异有统计学意义(P<0.05)。研究组患儿的优良率(98.3%)明显高于对照组(61.7%)，差异有统计学意义(P<0.05)。研究组无感染例数，对照组有2例恢复较差，出现感染，差异有统计学意义(P<0.05)。结论：采用闭合复位克氏针固定术治疗小儿肱骨髁上骨折的临床疗效较佳，患儿术后恢复情况的优良率较高，骨折部位愈合所需时间短，感染率低，无其他并发症，患儿家属普遍愿意接受，临床推广使用价值较高。

【Abstract】 Objective：To evaluate and analyze the clinical effect of closed reduction and Kirschner wire fixation on supracondylar fracture of the humerus in children.Method：According to the inclusion and exclusion criteria，120 cases of fracture of humeral condyle who achieved treatment in Pediatrics of our hospital from 2012 December to 2014 December were selected as clinical research object，and the children were randomly divided into study group and control group in accordance with the random number table method，each group had 60 cases.The control group used open reduction and Kirschner wire fixation，study group used closed reduction and Kirschner wire fixation.After treatment during 6-12 months，did visit investigation of two groups recovery，the difference of clinical effect was compared and made the score.Result：After supracondylar fracture of the humerus，return data displayed that study groups’ operation time，the time of hospitalization and bone fracture healing time compared with the control group were significantly decreased，there were statistical significance (P<0.05).Postoperative recovery of 30 patients in research group were classified as excellent，29 cases good，1 case general，14 cases were classified as excellent in control group，23 cases good，21 cases general，there was statistical significance (P<0.05).The excellent and good rate of study group of children (98.3%) was significantly higher than that in control group (61.7%)，the differences were obvious，with statistical significance (P<0.05).Research group of no infection cases，the control group had 2 cases recovery is poor，infection，the difference was statistically significant (P<0.05).Conclusion：The clinical curative effect of supracondylar fracture of humerus in children with closed reduction and Kirschner wire fixation is better，the excellent rate of postoperative recovery is higher，fracture healing time is short，with low infection rate and no other complications，the child’s family are generally willing to accept，it has high clinical value., 百拇医药(李现军)
