访谈型用药指导对2型糖尿病患者血糖相关指标的影响研究(1)
【摘要】 目的：探讨访谈型用药指导对2型糖尿病(T2DM)患者血糖相关指标的影响。方法：回顾性分析2013年1月-2014年12月在本院诊治200例T2DM患者的临床资料，采用随机数字表法将患者分为两组，每组各100例。对照组给予常规用药指导，观察组给予访谈型用药指导措施，比较两组患者的血糖情况。结果：干预前，两组患者的FBG、2 h BG及HbA1c水平比较差异无统计学意义(P>0.05)；干预1个月后，观察组指标分别为(5.82±0.42)mmol/L、(7.41±0.66)mmol/L、(5.38±0.32)%，对照组分别为(7.85±0.72)mmol/L、(9.63±0.64)mmol/L、(7.21±0.56)%，两组的血糖指标均较干预前明显改善，观察组患者改善程度明显优于对照组，差异有统计学意义(P<0.05)。干预前两组的用药知识掌握率分别为20.0%与16.0%，两组比较差异无统计学意义(P>0.05)；干预后两组的用药知识掌握率均较干预前明显改善，分别为96.0%与80.0%，观察组的改善情况明显高于对照组，两组比较差异有统计学意义(P<0.05)。结论：访谈型用药指导措施能够显著改善患者的血糖情况，提高患者的合理用药知晓率。

【Abstract】 Objective：To investigate the interview medication guide for type 2 diabetes mellitus (T2DM) index influencing the blood sugar.Method：The clinical data of diagnosis and treatment of 200 T2DM patients were retrospective analyzed from January 2013 to December 2014 in our hospital，and randomly divided into two groups according the method of random number table，100 cases in each group.The control group received routine medication guidance，the observation group was given interview medication guidance measures，the blood sugar level statistical of two groups were compared.

Result：Before the intervention，the difference of two groups had no statistically significant(P>0.05)；1 month after the intervention，the observation group index were(5.82±0.42)mmol/L，(7.41±0.66)mmol/L and (5.38±0.32)%，the control group were(7.85±0.72)mmol/L，(9.63±10.64)mmol/L and (7.21±0.56)%，blood glucose index of the two groups were compared with that before the intervention significantly improved，the patients in observation group were significantly better than the improved degree the control group，the differences were statistically significant(P<0.05).The two groups before the intervention of medication knowledge rates were 20% and 16%，with no significant difference(P>0.05)；intervention of two groups after medication knowledge rate than before the intervention significantly improved，respectively 96% and 80%，the improvment the situation of the observation group was significantly higher than that in the control group，the difference was statistically significant(P<0.05).Conclusion：The interview medication guidance measures can significantly improve blood sugar control of patients，improve the rational use of drugs in patients with known rate.

随着社会发展和人们生活水平的提高，糖尿病的发病率呈逐年升高的趋势，其中以2型糖尿病为主，约占糖尿病总数的90%左右[1]。随着对高血糖研究的深入，高血糖与降糖药物的研究发现，糖尿病患者用药不合理是导致糖尿病血糖控制不理想及增加并发症的风险[2]。因此，对糖尿病患者的治疗与用药指导应引起广大医务工作者的高度重视，访谈型用药指导是一种以患者为中心的用药指导方式，着眼于改变患者的治疗动机，以达到改变其生活习惯的目的[3]。在访谈教育的过程中，教育者主要是通过发现与妥善处理患者行为改变过程中的不良用药习惯，进而达到增强患者用药行为改变的内在动机，其对糖尿病患者的血糖控制及糖尿病并发症的防治具有重要价值[4]。本文探讨了访谈型用药指导在糖尿病患者治疗中的应用，旨在为糖尿病的用药指导方式提供一定的参考，总结如下。, http://www.100md.com(李竹岩)
