超低位直肠癌保肛手术应用效果评估分析(1)
【摘要】 目的：探讨超低位直肠癌保肛手术的临床应用效果。方法：选取本科2012年3月-2015年3月收治的90例直肠癌患者作为研究对象，按照随机数字表法将其分为研究组和对照组，每组45例。对照组实施传统开腹手术，研究组实施超低位直肠癌保肛手术，观察比较两组患者的手术指标、术后指标及并发症发生情况。结果：研究组的手术时间明显长于对照组，且术中出血量明显少于对照组，肛门排气时间、正常饮食时间及住院时间均明显短于对照组，差异均有统计学意义(P<0.05)。研究组的并发症发生率4.44%明显低于对照组的20.00%，差异有统计学意义(P<0.05)。结论：直肠癌患者实施超低位直肠癌保肛手术治疗尽管手术时间更长，但其术中出血量少，并发症发生率低，患者恢复更快，值得临床推广应用。

【Abstract】 Objective：To investigate the clinical effect of anus preserving operation for ultra-low rectal cancer.Method：90 patients with rectal cancer admitted to our department from March 2012 to March 2015 were selected as research objects，they were randomly divided into the research group and the control group，45 cases in each group.The control group was given traditional open surgery，the research group was given anus preserving operation for ultra-low rectal cancer.The operation indicator，postoperative indicators and occurrence of complications between the two groups were observed and compared.Result：The operative time of the research group was significantly longer than the control group，but intraoperative blood loss was significantly less than the control group，the anal exhaust time，normal diet time and hospitalization time were significantly shorter than the control group，the differences were statistically significant(P<0.05).The incidence of complications in the research group was 4.44%，which was significantly lower than 20.00% in the control group，the difference was statistically significant(P<0.05).Conclusion：The anus preserving operation for ultra-low rectal cancer in the treatment of patients with rectal cancer，although the operation time is longer，but intraoperative blood loss is less，the incidence of complications is low，the recovery of patients faster，is worthy of clinical popularization and application.

直肠癌属于临床常见疾病，从国内相关报告来看，其发病率逐年上升，对患者身心健康与生活质量都会产生影响，需加强重视[1]。低位直肠癌指的是与肛缘距离8 cm的直肠癌，其中若距离低于5 cm则视为超低位直肠癌[1]。对于直肠癌的治疗最终目标主要是局部控制肿瘤，以及提高患者的远期生存率。以往治疗超低位直肠癌患者以经腹会阴联合切除术为主[2]，但随着微创手术技术的发展与完善，近几年超低位直肠癌保肛术逐渐取而代之，显示可取得良好的效果。为了进一步探讨超低位直肠癌保肛手术的临床应用效果，本院进行了相关研究，现报告如下。, http://www.100md.com(杨明利等)
