脑电图与头颅CT相结合对新生儿缺氧缺血性脑病预后评估的研究(1)
【摘要】 目的:探讨脑电图联合头颅CT对新生儿缺氧缺血性脑病预后评估的应用价值。方法:回顾性分析本院2013年7月-2014年7月新生儿科住院的55例新生儿缺氧缺血性脑病患儿的临床资料,患儿在生后的3~5 d实施头颅CT与脑电图检查,将头颅CT结果、脑电图结果相结合,评估患儿预后,为临床干预和治疗提供依据。结果:55例患儿经脑电图检测,正常12例,轻度异常37例,重度异常6例。经头颅CT检测,正常14例,轻度6例,中度20例,重度15例。随访12个月,8例患儿出现后遗症,脑瘫5例,癫痫3例。结论:临床应用脑电图联合头颅CT对新生儿缺氧缺血性脑病进行诊断、早期确诊、评价脑损伤程度及预后,有助于提高患儿长期生存生活质量,值得广泛推广应用。

【Abstract】 Objective: To investigate the prognosis value of EEG combined with cranial CT for neonatal hypoxic-ischemic encephalopathy.Method:55 cases of neonatal hypoxic-ischemic encephalopathy in our hospital were selected from July 2013 to July 2014,their clinical data were retrospective analyzed. These children were given cranial CT and EEG after birth 3 to 5 days. The prognosis was assessed by the CT scan results combined with EEG results, the basis for clinical intervention and treatment were provided by them.Result:55 children detected by EEG, 12 cases of normal, 37 cases of mild abnormalities and 6 cases of severe abnormalities. By CT detected,14 cases of normal, 6 cases of mild, 20 cases of moderate and 15 cases of severe.Follow-up of 12 months, 8 children with complications, 5 cases of cerebral palsy, 3 cases of epilepsy. Conclusion: The clinical application of EEG combined with cranial CT on neonatal hypoxic-ischemic encephalopathy diagnosis, early diagnosis, evaluation and prognosis of the extent of brain damage, help to improve the quality of life in children with long-term survival, it is worth widely used.

临床研究结果显示,新生儿围产期由于缺氧引发脑组织缺氧缺血性损伤,临床称之为缺氧缺血性脑病(Hypoxic-ischemic encephalopathy,HIE),该病为新生儿常见病,病死率高,可达60%~80%,存活患儿预后效果差,出现神经系统后遗症[1]。本院从2013年7月开始研究应用脑电图联合头颅CT对新生儿缺氧缺血性脑病进行预后评估,现报道如下。

1 资料与方法
1.1 一般资料 选取本院从2013年7月-2014年7月新生儿科住院的55例新生儿缺氧缺血性脑病患儿做为研究对象。所有患儿均符合缺氧缺血性脑病的临床诊断标准:(1)有明确的可导致胎儿宫内窘迫的异常产科病史,以及严重的胎儿宫内窘迫表现(胎心<100次/min,持续5 min以上,和/或羊水Ⅲ度污染),或者在分娩过程中有明显窒息史;(2)出生时有重度窒息,指Apgar评分1 min≤3分,并延续至5 min时仍≤5分,或者出生时脐动脉血气pH≤7.00;(3)出生后不久出现神经系统症状、并持续至24 h以上[2]。上述患儿排除电解质紊乱、颅内出血和产伤等原因引起的抽搐以及宫内感染、遗传代谢性疾病和其他先天性疾病所引起的脑损伤。55例患儿中男28例,女27例;日龄1 h~7 d,平均(45.5±28.5)h;应用全国HIE临床诊断与分度标准对患儿实施分度,其中重度窒息8例,中度窒息23例,轻度窒息24例[3]。, 百拇医药(李德峰 胡小强 施彩嫦 武才胜)
