经阴道超声联合经腹部超声在宫外孕诊断方面的应用价值(1)
【摘要】 目的：探讨经腹部超声及经阴道超声联合使用在宫外孕诊断方面的应用价值。方法：收集2012年6月-2015年8月均经本院手术病理证实为宫外孕的185例患者为研究对象，所有患者均进行经阴道超声及经腹部超声检查，比较经腹部超声、经阴道超声及两种诊断方法联合使用的诊断正确率及附件区包块、卵黄囊、胎芽和胎心、盆腔积液检出率。结果：联合应用的诊断准确率明显高于经阴道超声及经腹部超声，经阴道超声的诊断准确率明显高于经腹部超声，差异均有统计学意义(P<0.05)。联合应用的附件区包块、卵黄囊、胎芽和胎心、盆腔积液检出率明显高于经阴道超声及经腹部超声，差异均有统计学意义(P<0.05)；经阴道超声附件区包块、卵黄囊、胎芽和胎心的检出率明显高于经腹部超声，差异有统计学意义(P<0.05)。结论：经腹部超声和经阴道超声两种检查方法联合应用能显著提高宫外孕患者的诊断正确率，效果明显优于单独应用经阴道超声或经腹部超声的检查结果，可在临床推广应用。

【Abstract】 Objective：To investigate the application value of transvaginal ultrasound combined with abdominal ultrasound in diagnosis of ectopic pregnancy.Method：185 patients with ectopic pregnancy in our hospital were selected from June 2012 to August 2015，they were ectopic pregnancy.All patients underwent transvaginal ultrasound and abdominal ultrasound，the diagnostic accuracy and the detection rate of the attachment area mass， yolk sac， fetal bud and fetal heart and pelvic effusion of three methods were compared.Result： The diagnostic accuracy rate of two diagnostic methods combined was significantly higher than the transvaginal ultrasound and abdominal ultrasound， the diagnostic accuracy rate of the transvaginal ultrasound was significantly higher than abdominal ultrasound， the differences were statistically significant(P<0.05). The detection rate of the attachment area mass， yolk sac， fetal bud and fetal heart and pelvic effusion in the combined application were significantly higher than that of transvaginal ultrasound and abdominal ultrasound，the differences were statistically significant(P<0.05). The detection rate of attachment area mass，yolk sac，fetal bud and fetal heart in transvaginal ultrasound were significantly higher than that of abdominal ultrasound， the differences were statistically significant(P<0.05).Conclusion：The abdominal ultrasound combined with transvaginal ultrasound can significantly improve the diagnostic accuracy in patients with ectopic pregnancy，better than alone transvaginal ultrasound or abdominal ultrasound examination result，it is worth in clinical application.

本研究中笔者用阴道超声联合腹部超声对来本院就诊的185例宫外孕患者进行诊断，与单独应用阴道超声、腹部超声对宫外孕诊断的结果进行对比分析，现报道如下。
1 资料与方法
1.1 一般资料 收集2012年6月-2015年8月来本院接受检查的宫外孕患者185例作为研究对象，均经手术病理证实为宫外孕。年龄21～35岁，平均(26.1±4.3)岁；就诊时154例患者有停经史，停经时间36～50 d，平均(44.7±7.5)d，其余的31例患者无明确停经史；婚姻状况：未婚33例，已婚152例；初产妇86例，经产妇99例，平均产次(1.7±0.3)次；其中输卵管伞部妊娠98例，输卵管峡部妊娠47例，输卵管间质部妊娠30例，卵巢妊娠10例；症状表现：下腹痛45例，少量阴道流血72例，腹痛伴有阴道流血58例，无症状10例。, http://www.100md.com(汤华 姚惠莉 陈红方 艾利)
