血肌钙蛋白和心肌酶谱检测用于早期预测儿童重症肺炎发生的临床研究(1)

     【摘要】 目的:探索研究血肌钙蛋白和心肌酶谱检测用于早期预测儿童重症肺炎发生的临床意义。方法:选取本院就诊的50例患儿分为重症肺炎组17例和轻症肺炎组33例,另选取在本院正常体检的儿童30例作为对照组,肺炎患儿在入院时行心肌酶谱检测和心肌肌钙蛋白检测,之后给予相对应的抗感染、止咳、平喘等对症治疗。患儿肺炎痊愈后再进行1次心肌酶谱、心肌肌钙蛋白检测,采用对照组儿童体检血液进行心肌钙蛋白和心肌酶谱检测,比较治疗前后各组的心肌钙蛋白和心肌酶谱情况。结果:治疗前检测重症肺炎及轻症肺炎组的肌钙蛋白及心肌酶谱均高于对照组,差异均有统计学意义(P<0.05);治疗后重症肺炎组与轻症肺炎组的肌钙蛋白及心肌酶谱比较差异均无统计学意义(P>0.05)。结论:部分肺炎患儿合并有心肌损伤,对肺炎患儿行肌钙蛋白及心肌酶谱检测有助于判别是否有心肌损伤,及时采取综合治疗,可降低患儿死亡率,提高治愈率。

    【Abstract】 Objective: To explore the clinical significance of serum cardiac troponin and myocardial enzyme spectrum in the early prediction of severe pneumonia in children.Method: 50 cases with pneumonia in our hospital were divided into severe pneumonia group for 17 cases and mild pneumonia group for 33 cases, another 30 children with normal physical examination in the hospital were selected as the control group.Children with pneumonia at the time of admission were given cardiac zymography and cardiac troponin testing,then they were treated with the corresponding anti-infection, cough, asthma and other symptomatic treatment. Patients with pneumonia after recovery and the control group’s children were given the myocardial enzyme spectrum and cardiac troponin test.The cardiac troponin and myocardial enzymes at pre-treatment and post-treatment of the three groups were compared.Result:Before treatment,the serum cardiac troponin and myocardial enzyme spectrum of severe pneumonia group and mild pneumonia group were higher than the control group,the differences were statistically significant(P<0.05);after treatment,compared severe pneumonia group and mild pneumonia group of those, the differences were not statistically significant(P>0.05).Conclusion: Part of pneumonia in children with myocardial injury, the children with pneumonia and myocardial enzyme spectrum detection can help determine whether there is myocardial injury,and timely comprehensive treatment, it can reduce the mortality rate of children and improve the cure rate.

    肺炎是小儿呼吸系统的常见病、多发病,如不能有效控制,可对小儿造成较严重危害,甚至导致死亡。而重症肺炎常伴随有神经系统、心脏、肾脏损伤等并发症,其中心肌损伤较为常见,也是危及生命的主要并发症。在原发病的肺炎治疗之外,寻找合理的生化检测指标来提高早期诊断重症的可靠性及对患儿预后的指导性,从而早期发现减少合并心肌损伤的重症肺炎患儿的并发症发生率和病死率。本文通过分析比较轻重症肺炎患儿的血肌钙蛋白和心肌酶谱关系,从而早期预测重症肺炎的发生,以进行早期干预,改善预后[1-3]。本研究对50例肺炎患儿通过检测肌钙蛋白和心肌酶谱进行比较,现将结果汇报如下。, 百拇医药(虞致新 华锋 张清秀)
