前牙区骨劈开同期种植修复1年后软组织美学效果评价(3)
本研究观察的16枚种植体中PES最高得分为14，最低为6，均值为(10.41±2.09)，其中6分出现2次，14分出现1次，10分和11分出现次数最多，说明较差的病例和完美的病例均较少，大部分修复的效果基本令人满意。由于样本量较小，仅有16个有效的PES得分进行统计，因此，本次研究中PES标准差较大，为2.09，数据比较离散。从表2中可以看出近中牙乳头和软组织形态的平均得分最低，分别为(1.17±0.86)和(1.17±0.73)。这与国内外不同学者研究的结果存在一定差异[2，10，13]，说明种植术后软组织的丧失仍是不可避免的问题，种植义齿牙龈乳头及形态存在很大的个体差异，种植义齿修复软组织重建存在一定的难度。Becker等[14]指出薄龈生物型相对厚龈生物型在缺牙后更易发生牙龈退缩及龈乳头缺如，因此，患者牙周组织类型也会影响其PES得分。本研究中有7例患者为薄龈生物型，其PES评分中近远中龈乳头、软组织水平、软组织形态得分均相对较低，也证实了这一点。在今后的手术中可通过带蒂黏膜瓣转移、上皮下结缔组织移植和自体游离龈移植等进行软组织增量。Gehrke等[15]研究表明不同专业的评分者对PES得分影响较大，在运用PES评分标准进行评价时，应注意这一点。
