音乐疗法对乳腺癌患者抑郁影响的Meta分析(1)
【摘要】 目的：系统评价音乐疗法对乳腺癌患者抑郁的有效性。方法：检索2004年1月1日-2014年5月1日公开发表在PubMed、Embase、Cochrane Library、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CMB)、维普(VIP)及万方文献数据库的音乐疗法治疗乳腺癌患者抑郁的随机对照试验(RCT)，应用抑郁自评量表(SDS)对两组干预前和干预后的抑郁进行评价，并用RevMan 5.2软件进行Meta分析。结果：共有8篇文献547例乳腺癌被纳入分析，其中音乐疗法组273例，常规护理组274例。音乐疗法亚组分析，音乐疗法干预前两组的乳腺癌抑郁比较差异无统计学意义(WMD=-0.20，95%CI：-2.74～2.34，Z=0.1，P>0.05)；而音乐疗法干预后的乳腺癌抑郁明显改善，差异有统计学意义(WMD=-4.49，95%CI：-7.21～-1.77，Z=3.23，P<0.05)，两组数据汇总后发现音乐疗法明显改善患者的乳腺癌抑郁情况(WMD=-2.37，95% CI：-4.43～-0.37，Z=2.26，P<0.05)。结论：音乐疗法治疗乳腺癌患者抑郁的长期疗效优于常规护理。

【Abstract】 Objective： To evaluate the feasibility of music therapy in patients with breast cancer depression.Method：Literature search was performed in PubMed，Embase，Cochrane Library， CNKI， CMB，VIP and Wan fang database From January 1 2004 to May 1 2014 of music therapy for breast cancer patients with depression of randomized controlled trial (RCT). Two groups were investigated with depression self-rating scale (SDS) of Pre-intervention and posto-intervention.Meta-analyses were conducted in RevMan 5.2 software.Result：8 articles of 547 patients with breast cancer were included in this study. There were 273 patients in music therapy group and 274 patients in the conventional nursing group. In the sub-analysis of music therapy， there was not statistical difference of pre-intervention breast cancer depression in two groups(weighted mean difference； WMD=-0.20，95%CI：-2.74～2.34，Z=0.1，P>0.05)；the breast cancer depression incidence of post-intervention improved significantly than pre-intervention，the difference was statistically significant (WMD=-4.49，95%CI：-7.21～-1.77，Z=3.23，P<0.05).The music therapy in patients with breast cancer depression improved significantly (WMD=-2.37，95%CI：-4.43～-0.37，Z=2.26，P<0.05).Conclusion： Compared to conventional nursing，long-term clinical outcomes after music therapy in patients with breast cancer Depression are favorable.

抑郁症是乳腺癌患者手术前后最常见的负性心理反应之一，不仅严重影响患者的康复，而且还降低了患者的生存质量，并影响了乳腺癌的治疗效果[1]。为了解决乳腺癌患者手术前后存在严重的抑郁情绪，护理人员的有效干预极其需要。目前对乳腺癌患者抑郁的护理干预主要有心理干预、认知、行为治疗、音乐疗法等[2]。随着多项临床对照研究的开展，音乐疗法(Music therapy，MT)作为一种方便有效的心理干预手段用于乳腺癌患者抑郁的临床护理工作当中[3-4]。然而国内外研究资料样本量都不大，音乐疗法对乳腺癌患者抑郁的临床效果目前尚未有统一的观点。本文通过对国内外近十年来公开发表的有关音乐疗法对乳腺癌患者抑郁影响的文献进行系统分析，并且对音乐疗法的有效性进行评价，明确音乐疗法是否能很好控制乳腺癌患者抑郁，为改善乳腺癌患者的生活质量提供依据。, http://www.100md.com(杨方武 李祥龙 刘涛)
