从病因学角度论糖脂代谢紊乱对多囊卵巢综合征发病的影响(1)
【摘要】 目的:探究糖脂代谢紊乱与多囊卵巢综合征发病的相关性。方法:选择2013年1月-2014年12月本院收治的PCOS患者106例作为PCOS组,同期女性非PCOS患者106例作为对照组,采集患者清晨空腹静脉血并进行口服糖耐量试验。检测入选两组患者血糖(Glu)、胰岛素(INS)、卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、雄烯二酮(A)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)和高密度脂蛋白(HDL-C)水平。分析糖脂代谢水平对多囊卵巢综合征的影响。结果:PCOS组糖、脂代谢紊乱患者均多于对照组,比较差异均有统计学意义(P<0.05)。PCOS组患者伴随着糖代谢紊乱程度增加脂代谢紊乱加剧,比较差异均有统计学意义(P<0.05)。随着患者糖脂代谢紊乱程度加剧,PCOS患者T水平升高、月经紊乱程度加剧、卵巢多囊样改变患者比例升高,比较差异均有统计学意义(P<0.05)。结论:PCOS患者的糖代谢水平与脂代谢水平存在显著正相关性,糖脂代谢紊乱对PCOS疾病的发生具有重要推动作用或可能为其发病机制之一。

【Abstract】 Objective:To explore the correlation between glucose and lipid metabolism disorder and paroxysm of polycystic ovarian syndrome.Method:106 polycystic ovarian syndrome(PCOS) patients in our hospital from January 2013 to December 2014 were selected as the PCOS group,and 106 female patients without PCOS at the corresponding period were selected as the control group.The patients of the two groups were gathered the morning fasting venous blood and given oral glucose tolerance test.The levels of blood glucose,insulin,follicle-stimulating hormone,luteinizing hormone,testosterone,androstenedione,triglyceride,total cholesterol,low density lipoprotein and high density lipoprotein were detected.The correlation between glucose and lipid metabolism disorder and paroxysm of PCOS was analyzed.Result:The numbers of patients with dysglycemia and dyslipidemia in the PCOS group were more than those of the control group,the differences were statistically significant(P<0.05).In PCOS patients,with the increase of the degree of disordered glycometabolism,the disordered lipid metabolism increased,the differences were statistically significant(P<0.05).The androgen levels,the degree of paramenia and the proportion of patients with polycystic ovary increased with the aggravation of glucose and lipid metabolism disorder,the differences were statistically significant(P<0.05).Conclusion:There was a significantly positive correlation between the level of glycometabolism and the level of lipid metabolism.Glucose and lipid metabolism disorder has great promoting effects on the paroxysm of PCOS and may be one of the pathogenesis of PCOS.
