不同型号导尿管对长期留置尿管患者尿路感染的影响(1)
【摘要】 目的：比较不同型号导尿管对长期留置尿管患者尿路感染的影响。方法：选取2013年1月-2015年
3月本院长期留置导管患者293例，按随机数字表法分为小型号组97例、常规型号组101例和序贯组95例，分别给予12～14型号、16～18型号及初选适宜的最小型号，之后序贯依次递增型号的双腔气囊尿管，比较三组的尿道损伤、尿路感率、尿液溢出、尿管脱出、拔管后尿路刺激征的发生情况，并评价三组的留置尿管期间舒适度。结果：序贯组的尿道损伤、尿路感率、尿液溢出、拔管后尿路刺激征发生率均明显低于小型号组和常规型号组，差异均有统计学意义(P<0.05)；小型号组的尿路感染发生率明显低于常规型号组，但尿液溢出和尿管脱出的发生率明显高于常规型号组和序贯组，差异均有统计学意义(P<0.05)；小型号组和常规型号组的尿道损伤、拔管后尿路刺激征比较差异均无统计学意义(P>0.05)。序贯组和小型号组的舒适度均明显优于常规型号组，差异均有统计学意义(P<0.05)，序贯组和小型号组的舒适度比较差异无统计学意义(P>0.05)。结论：初选适宜的最小型号，之后序贯依次递增型号的双腔气囊尿管可有效预防和减少长期留置尿管患者尿路感染的发生，增加患者舒适度，值得临床推广应用。

【Abstract】 Objective：To discuss the influence of different types of catheter on the urinary tract infection for patients with indwelling catheter long term. Method：293 patients with indwelling catheter long term in our hospital were selected from January 2013 to March 2015， they were randomly divided into small model group of 97 cases， conventional model group of 101 cases and sequential group of 95 cases.The small model group was given 12-14 model， conventional model group was given 16-18 model and sequential group was given the smallest model at first choice，sequential increasing model of double cavity balloon catheter followed.The urethral injury，urinary tract infection， urine overflow， urinary catheter and urinary tract irritation after extubation of three groups were compared. The comfort during the period of indwelling catheter of three groups were evaluated.Result：The rate of urethral injury， urinary tract infection， urine overflow and urinary tract irritation after extubation of sequential group were significantly lower than those of small model group and conventional model group，the differences were statistically significant(P<0.05).The rate of urinary tract infection of small model group was significantly lower than that of conventional model group，but the rate of urinary injury and urine overflow were significantly higher than those of sequential group and conventional model group， the differences were statistically significant(P<0.05).There was not significant difference of the rate of urethral injury and urinary tract irritation after extubation between the small model group and conventional model group(P>0.05). The comfort level of the sequential group and the small group was significantly better than that of the conventional model group， the difference was statistically significant(P<0.05)， and there was not significant difference between the sequential group and the model group(P>0.05).Conclusion：First choice the smallest model， sequential increasing model of double cavity balloon catheter followed can prevent and reduce urinary tract infection of patients with indwelling catheter long term， increase the comfort，it is worthy of clinical application., 百拇医药(陈伟焕 卢慧清)
