经皮长尾可折椎弓根螺钉内固定治疗胸腰椎骨折临床疗效分析(1)
【摘要】 目的：观察经皮长尾可折椎弓根螺钉内固定治疗胸腰椎骨折的临床疗效。方法：随机选择2008年2月-2015年8月本院收治的行胸腰椎骨折治疗的患者70例，采用随机数字表法将患者分为观察组和对照组，每组35例。观察组使用微创治疗(经皮长尾可折椎弓根螺钉内固定)，对照组使用开放式手术治疗，对两组患者手术相关指标、术前、术后3 d，术后3周以及术后半年的VAS评分进行比较，并对比两组患者手术前后椎体前缘高度、矢状面指数以及Cobb角的情况，以观察两种术式对患者的影响。结果：对两组患者手术相关指标进行比较后发现，观察组患者在切口长度、手术时间、引流量、出血量以及住院时间等方面均优于对照组，比较差异均有统计学意义(P<0.05)；两组患者术前与术后6个月VAS评分无统计学差异(P>0.05)，术后3 d、3周以及术后3个月观察组VAS评分均低于对照组，比较差异均有统计学意义(P<0.05)；术后两组患者椎体前缘高度、矢状面指数和Cobb角等指标均较术前有所改善，比较差异均均有统计学意义(P<0.05)。结论：经皮长尾可折椎弓根螺钉内固定治疗胸腰椎骨折效果确切，能够缩短治疗时间，减轻患者痛苦。

【Abstract】 Objective：To observe and analyze the clinical curative effect of percutaneous long tail foldable pedicle screw intemal fixator treatment for thoracolumbar fractures.Method：70 cases of thoracolumbar fractures were divided into the observation group and the control group according to the method of throwing random in our hospital from February 2008 to August 2015，each group had 35 cases.The observation group was treated with minimally invasive treatment and the control group was treated with open surgery，the effect of two kinds of operative methods for patients was observed，the indications related to the operation of the two groups，the VAS score of the patients preoperative and postoperative 3 days，3 weeks and six months，before and after the surgery spine flange height，sagittal index and Cobb Angle of two groups were compared.Result：The indicators related to the operation of two groups such as incision length，operative time，volume of drainage，blood loss and hospital stay of the observation group were better than those of control group，the differences were statistically significant(P<0.05).Preoperative and postoperative 6 months VAS score of two groups had no statistical differences(P>0.05)，postoperative 3 d，3 weeks and 3 months VAS scores of observation group were lower than the control group，the differences were statistically significant(P<0.05).Two groups of patients with postoperative fanterior flange，height sagittal index and Cobb Angle indexes improved，the differences were statistically significant(P<0.05).Conclusion：Percutaneous long tail foldable pedicle screw intemal fixator treatment for thoracolumbar fractures has exact effect，it can shorten the treatment time amd reduce patient’s pain., 百拇医药(林伟鹏 邹仲兵 叶劲 等)
