骨劈开、骨挤压术及GBR技术在前牙美学区种植的临床应用研究(1)

     【摘要】 目的:探讨骨劈开、骨挤压术、GBR技术不同的联合应用方法在前牙美学区种植的临床疗效。方法:随机选择上颌前牙单牙缺失患者72例,分别采取骨劈开联合骨挤压术(A组20例)、骨劈开联合GBR技术(B组25例)、骨劈开挤压术联合GBR技术(C组27例)进行种植修复。比较三组术后24个月的种植成功率、术前与术后24个月的牙槽嵴宽度增加值及修复完成1 h后与术后24个月的软组织美学效果[红色美学(PES)指数]变化情况。结果:(1)种植成功率:A、B、C三组术后24个月的种植成功率分别为100%、100%与96.30%,比较差异无统计学意义(P>0.05);(2)牙槽嵴宽度增加值:三组比较差异有统计学意义(P<0.05),其中C组优于A、B组(P<0.05),B组优于A组(P<0.05);(3)软组织美学效果:三组的PES变化百分率比较差异有统计学意义(P<0.05),其中C组最优,B组与A组比较差异无统计学意义(P>0.05)。结论:骨劈开、骨挤压术及GBR技术联合应用可以互相弥补其不足,提高临床疗效;前牙美学区应用骨劈开挤压术联合GBR技术在增加牙槽嵴宽度与软组织美学效果方面优于骨劈开联合骨挤压术及骨劈开联合GBR技术。

    【Abstract】 Objective:To explore the clinical curative effect of different combined application methods,including bone splitting technique,osteotome technique and guided bone regeneration technique for implanting in anterior teeth aesthetic region.Method:A Total of 72 cases of maxillary anterior single tooth loss were randomly selected,bone splitting combined with bone condensing technique (20 cases of group A),bone splitting combined with guided bone regeneration technique (25 cases of group B) and bone splitting combined with bone condensing and guided bone regeneration technique (27 cases of group C) were taken respectively for implant rehabilitation.The successful rates of implants 24 months after secondary surgery,the added value of alveolar ridge width of preoperative and postoperative 24 months,percentage change of soft tissue aesthetic effect [pink esthetic score (PES) index] between 1 h after repair of complete and postoperative 24 months in three groups were compared.Result:(1)The success rate of implants:The success rate of implants 24 months after secondary surgery in the group A,B,C were 100%,100% and 96.30% respectively,there were no statistical differences(P>0.05).(2)The added value of alveolar ridge width:The added value of alveolar ridge width of the three groups were statistically significant differences(P<0.05),the group C was significantly better than that of A, B group(P<0.05),the group B was significantly better than that of the group A(P<0.05).(3)Soft tissue aesthetic effect:percentage change of PES index was the best in the group C (P<0.05),there was no significant differences between group A and B(P>0.05).Conclusion:Combination of bone splitting technique,osteotome technique and guided bone regeneration technique can make up for the deficiency of each other,improve the clinical curative effect.Applying the bone splitting combined with bone condensing and guided bone regeneration technique are better than that applying bone splitting combined with bone condensing technique or bone splitting combined with guided bone regeneration technique,as far as the the added value of alveolar ridge width,soft tissue aesthetic effect in anterior teeth aesthetic region., 百拇医药(范静)
