系统化整体护理干预在开放性骨折感染防治中的应用(1)
【摘要】 目的：探讨系统化整体护理干预在开放性骨折感染防治中的应用，以期为优化开放性骨折临床护理提供参考依据。方法：选取2014年3月-2015年10月本院收治的开放性骨折患者96例，根据患者所接受的护理方式分为观察组(n=52)和对照组(n=44)，对照组患者予以封闭负压引流技术预防感染，同时接受骨折常规护理，观察组患者则在对照组的基础上实施系统化整体护理干预，比较两组患者感染情况，评估患者生活质量，并于患者出院前1 d发放护理满意度调查表，统计患者对护理的满意情况。结果：观察组5例患者发生感染，感染率为9.62%，对照组11例患者发生感染，感染率为25.00%，两组比较差异有统计学意义(P<0.05)；护理干预后，观察组患者总体健康、心理健康、生理功能、生理职能、社会功能、躯体疼痛、精力、情感职能得分均明显高于对照组患者(P<0.05)；护理干预后，观察组患者对护理的满意度明显高于对照组患者(P<0.05)。结论：对开放性骨折患者实施系统化整体护理干预进行预防感染，可显著降低感染率，改善患者生活质量，提高护理质量，值得临床推广。

【Abstract】 Objective：To discuss the application of systematic holistic nursing intervention in the prevention and treatment of open fractures infection，in order to provide reference for the optimization of clinical nursing.Method：A total of 96 cases of patients with open fractures in our hospital from March 2014 to October 2015 were selected and divided into the observation group and the control group，patients in the control group were treated with closed negative pressure drainage，and accepted fracture routine nursing，the observation group was given systematic holistic nursing intervention on the basis of the control group.The occurrence of infection，quality of life，nursing satisfaction of two groups were compared.Result：The infection rate of the observation group was 9.62%，and the control group was 25.00%，there was statistically significant difference(P<0.05).After nursing intervention，the score of overall health，mental health，physical function，physical function，social function，physical pain，energy，emotional function of the observation group were significantly higher than those of the control group(P<0.05).After nursing intervention，the nursing satisfaction of the observation group was significantly higher than that of the control group(P<0.05).Conclusion：Implementing systematic holistic nursing intervention for prevention of open fractures infection，can significantly reduce the rate of infection，improve the quality of life and the quality of nursing，it is worthy of promotion.

创面感染是开放性骨折患者的常见并发症之一，可对治疗和康复造成不良影响，尤其是部分病情严重、体质虚弱者，是目前骨科临床治疗较棘手的问题[1]。封闭负压引流技术是一种新型的创面治疗技术，该技术通过应用生物透性膜封闭创面以避免创面与外界接触，进而减少创面接触病原微生物的可能，同时还通过高负压状态清除引流区渗出液和坏死组织，有助于创面愈合[2-3]。多年临床实践发现，优质护理干预可有效降低骨科创伤患者感染率，然而开放性骨折患者经封闭负压引流技术治疗尚无规范化、标准化的护理模式[4]。因此，本研究对开放性骨折患者实施系统化整体护理进行预防感染，取得较满意的护理效果，现报道如下。, 百拇医药(钟德霞 席晓颖 曾伟清)
