基于护士镇静管理模式对机械通气患儿的影响研究(1)
【摘要】 目的：探討基于护士镇静管理模式对机械通气患儿的影响。方法：将2014年1月-2015年12月在东莞市人民医院住院的接受机械通气及镇静治疗的患儿30例作为研究对象。按照入院顺序抽签后随机分为试验组和对照组，对照组采用以医生为主体的镇静模式，试验组采用护士的镇静管理模式，观察患儿行为认知、术后24 h韦氏量表评分、血气、循环、呼吸、镇静效果、机械通气时间、ICU住院时间、总住院时间。结果：试验组患儿言语陈述时间、定向力恢复时间均低于对照组，差异均有统计学意义(P<0.05)；试验组术后24 h韦氏量表评分迷津值、译码值均高于对照组，差异均有统计学意义(P<0.05)；试验组、对照组治疗后PaO2、MAP、呼吸、HR、Ramsay评分均低于同组治疗前，差异均有统计学意义(P<0.05)。试验组机械通气时间、ICU住院时间、总住院时间均低于对照组，差异均有统计学意义(P<0.05)。结论：基于护士镇静管理模式对机械通气患儿有积极的影响，患儿言语陈述时间、定向力恢复时间较短，迷津值、译码值较高，PaO2、MAP、呼吸、HR、Ramsay评分恢复较好，机械通气时间、ICU住院时间、总住院时间较短，临床应用价值较高。

【Abstract】 Objective：To explore the influence on the children with mechanical ventilation by sedation management mode on the basis of nurse.Method：A total of 30 cases of patients with mechanical ventilation were randomly divided into the experiment group and the control group，the control group was cared by doctors as the main body to be calm，the experiment group was cared by sedation management mode on the basis of nurse，the cognitive behavioral，the scores of wechsler scales after operation for 24 h，blood gas，circular，respiratory，calm effect，the duration of ventilatory support，ICU hospital stays，the total length of hospital stay of two groups were observed.Result：The speech time，directional force recovery time of the experiment group were lower than those of the control group(P<0.05).The maze value，decoding value for the experiment group were higher than those of the control group(P<0.05).The PaO2，MAP，respiratory，HR，Ramsay of two groups after treatment were lower than those of before treatment(P<0.05).The duration of ventilatory support，ICU hospital stays，the total length of hospital stay of the experiment group were lower than those of the control group(P<0.05).Conclusion：It has active influence on the children with mechanical ventilation by sedation management mode on the basis of nurse，with short speech time，directional force recovery time，with high maze value，decoding value，it has good recover of PaO2，MAP，respiratory，HR， Ramsay，with short of duration of ventilatory support，ICU hospital stays，the total length of hospital stay，it has high values of clinical application.
