雾化吸入PGE1治疗支气管哮喘急性发作对血气分析的影响(1)
【摘要】 目的:探讨雾化吸入前列腺素E1(PGE1)治疗支气管哮喘急性发作期对血气分析水平的影响。方法:选取本院2015年1-12月的50例支气管哮喘急性发作期患者为研究对象,将50例支气管哮喘急性发作期患者随机分为两组,其中对照组24例患者全身应用解痉、平喘、抗感染等常规综合治疗,治疗组26例患者在常规综合治疗的基础上加用雾化吸入PGE1治疗,并比较两组治疗3 d后患者的桡动脉血检测PaO2、PaCO2指标的变化。结果:治疗组患者经过治疗后总有效率为96.15%,明显高于对照组的67.67%,差异有统计学意义(P<0.05)。治疗后,治疗组的PaO2、PaCO2均明显优于对照组,差异均有统计学意义(P<0.05)。结论:加用雾化吸入PGE1治疗支气管哮喘急性发作期临床效果显著,大大的减少了患者的痛苦,提高了患者的生活质量,值得在临床中推广应用。

Influence on Blood Gas Analysis of Atomization Inhalation of PGE1 in the Treatment of Bronchial Asthma in Acute Phase/SONG Wei-rong,ZHAO Hua,LIU Chang.//Medical Innovation of China,2017,14(03):132-135
【Abstract】 Objective:To investigate the effect of inhaled prostaglandin E1(PGE1) on blood gas analysis in acute exacerbation of bronchial asthma.Method:50 patients with bronchial asthma in acute phase of our hospital from January 2015 to December 2015 were selected as the research objects,the patients were divided into two groups,the control group (24 patients) and treatment group (26 patients).The control group applied conventional comprehensive treatment such as systemic spasmolysis,relieving asthma,anti-inflammatory,in treatment group who were given routine treatment combined with atomization inhalation of PGE1.Then compared the patient’s radial artery blood PaO2,PaCO2 index changes between the two groups after treatment 3 days.Result:After treatment,the total effective rate of the treatment group was 96.15%,which was significantly higher than 67.67% of the control group,the difference was statistically significant(P<0.05).After treatment,the PaO2 and PaCO2 in the treatment group were significantly better than the control group,the differences were statistically significant(P<0.05).Conclusion:The clinical effect of inhalation of PGE1 in the treatment of acute exacerbation of bronchial asthma is significant, greatly reducing the pain of patients,improve the quality of life of patients,it is worthy of clinical application.

支气管哮喘又被简称为哮喘,是因过敏原或者其他非过敏性因素引起的一种支气管反应性极度增高的过敏性疾病[1-2]。该病已经非常严重的影响了人们的身体健康,给人们的日常生活带来了巨大的痛苦。因此,针对该病的诊断、治疗以及病因病机的研究和探讨等,越来越多的引起了社会上广大医务工作者的重视。
支气管哮喘可以诱发肺通气和/或换气功能严重障碍,以致不能进行有效的气体交换,导致缺氧伴或不伴二氧化碳潴留,从而引起一系列生理功能和代谢紊乱的临床综合征。文献[3]研究表明,布地奈德、沙丁胺醇联合噻托溴铵对老年支气管哮喘能夠起到很好的治疗效果,改善患者的血气分析指标并有效缓解相关临床痒状、体征。前列腺素E1(PGE1)能使血管平滑肌松弛,从而减少血流的外周阻力,降低血压。因此,本研究选取患者的血气分析作为指标,观察患者治疗前后桡动脉血中PaO2、PaCO2的变化。PGE1是常用的治疗支气管哮喘的药物,它能够抑制多种参与发病的脱敏细胞因子和黏附分子的生成以及抑制免疫系统功能。PGE1具有扩血管、抗炎、抗血小板聚集、保护血管内皮细胞、防止动脉粥样硬化等作用[4]。近年来,临床研究发现,该药尚有不少新用途,其可以用于治疗支气管哮喘。PGE1是前列腺素中具有很强舒张支气管作用的物质,其作用机制是可直接对抗血栓素的收缩支气管作用,并且维持内皮细胞功能等作用[5]。本研究着眼于前列腺素E1对支气管哮喘的治疗作用,从而开辟该病治疗领域的新方向。, http://www.100md.com(宋威蓉 赵华 刘昌)
