感染性休克时应用FOCUSED TEE监测心血管循环功能的临床价值探讨(1)
【摘要】 目的：探讨经食道超声心动图(FOUCESD TEE)在围手术期发生感染性休克时心血管循环功能监测对临床治疗效果的影响。方法：选取2015年1月-2016年2月本院收治围手术期发生感染性休克患者60例作為研究对象，按照随机数字表法分为观察组与对照组，各30例。所有患者均采用监测血压、中心静脉压，行血气分析并监测患者周围循环同时记录尿量，结合以上数据实时早期目标导向性治疗；观察组加用FOUCESD TEE、对照组采用颈胸超声心动图(Transthoracic Echocardiography，TTE)连续观察患者心脏收缩舒张功能及前、后负荷，及时进行治疗方案的调整。观察比较两个组患者复苏情况、心功能、血气分析、血乳酸水平及尿量。结果：治疗后两组患者复苏情况、心功能、血气分析、血乳酸水平及尿量均有显著改善，观察组治疗效果显著优于对照组(P<0.05)。结论：经食道超声心动图可通过连续可视化监测心脏功能，计算评估外周循环阻力，对围手术期感染性休克患者的及时治疗起到积极作用，同时其具有创伤性小、操作性高等特点，能够有效提高感染性休克患者的临床治疗预后。

【Abstract】 Objective：To investigate the effect of Transesophageal Echocardiography(FOUCESD TEE) on cardiovascular function during perioperative period in patients with septic shock.Method：A total of 60 patients with septic shock during perioperative period in our hospital from January 2015 to February 2016 were selected and randomly divided into the observation group and the control group，30 cases in each group.All patients were treated by monitoring blood pressure，central venous pressure，blood gas analysis and record the amount of urine monitoring in patients with peripheral circulation，combined with the above data of early goal-directed therapy.The observation group added systolic and diastolic function before and after loading and continuous observation with FOUCESD TEE in patients with heart， timely adjustment of the therapeutic regimen，while the control group was treated with transthoracic echocardiography(TTE).The recovery，cardiac function，blood gas analysis，blood lactic acid level and urine output of two groups were observed and compared.Result：Resuscitation，cardiac function，blood gas analysis，blood lactate levels and urine volume of two groups were significantly improved，after treatment，the effect of the observation group was significantly better than that of the control group(P<0.05).Conclusion：Transesophageal echocardiography cardiac function by continuous visual monitoring and evaluation of peripheral vascular resistance，positive effect on the treatment of perioperative patients with septic shock，at the same time it has less invasive，higher operating characteristics，can effectively improve the clinical prognosis of patients with septic shock.
