缝合止血在卵巢囊肿手术中应用对患者免疫系统及卵巢功能的影响研究(1)
【摘要】 目的：探討缝合止血在卵巢囊肿手术中的应用效果及对患者免疫系统的影响。方法：回顾性分析2014年2月-2016年6月在本院接受手术治疗的卵巢囊肿患者的临床资料，根据其术中止血方式分为电凝组45例和缝合组40例。观察两组患者手术前后炎症细胞因子、免疫球蛋白水平的变化，比较两组患者手术前后性激素水平的差异。结果：两组患者手术前IL-2、IL-4和hs-CRP水平比较差异均无统计学意义(P>0.05)；手术后3 d两组患者的上述指标均较手术前升高，且电凝组增高更明显(P<0.05)。两组患者手术前IgG、IgA和IgM水平比较差异均无统计学意义(P>0.05)；手术后7 d时，两组患者的IgA和IgM水平均较手术前差异均无统计学意义(P>0.05)，而IgG较手术前降低，且电凝组降低更明显(P<0.05)。两组患者手术前FSH、LH和E2水平差异均无统计学意义(P>0.05)；手术后3个月，电凝组患者的FSH较手术前增高(P<0.05)，LH和E2水平较手术前降低(P<0.05)，而缝合组仅E2水平较手术前降低(P<0.05)，FSH和LH水平较手术前无明显变化(P>0.05)。结论：缝合止血在卵巢囊肿手术中的应用对患者的免疫系统和卵巢功能影响较小，具有临床推广应用价值。

【Abstract】 Objective：To investigate the effect of suture hemostasis in the operation of ovarian cyst and its effect on the immune system.Method：The clinical data of patients with ovarian cyst undergoing surgical treatment in our hospital from February 2014 to June 2016 were analyzed retrospectively.The changes of inflammatory cytokines and immunoglobulin levels before and after operation in two groups were observed，and the difference of sex hormone level before and after operation in two groups were compared.Result：There were no significant differences in IL-2， IL-4 and hs-CRP levels between the two groups before and after operation(P>0.05).The above indexes of the patients in the two groups were significantly higher than those before the operation，and the increase of the coagulation group was more obvious(P<0.05).Two groups of patients before surgery IgG， IgA and IgM levels without significant difference(P>0.05)；7 days after operation，two groups of patients with IgA and IgM levels were compared to preoperative no obvious change and IgG before surgery reduce and electrocoagulation group reduced more significantly(P<0.05).Two groups of patients before surgery FSH，LH and E2 levels without significant difference(P>0.05).3 months after the operation，electric coagulation group of patients with FSH than before the surgery increased(P<0.05)，LH and E2 levels were lower than those before operation(P<0.05)，and suture group only E2 level than before the surgery to reduce(P<0.05)，FSH and LH levels than before the surgery without significant changes(P>0.05).Conclusion：The application of suture hemostasis in the operation of ovarian cyst has little effect on the immune system and ovarian function，it has the value of clinical application., http://www.100md.com(林宏 黎婧琳 袁丽萍)
