呼气一氧化氮及肺功能监测在儿童哮喘中的应用(1)
【摘要】 目的:將呼气一氧化氮(fractional exhaled nitric oxide,FeNO)浓度与肺功能指标进行比较,评估两者在儿童支气管哮喘诊断中的应用价值。方法:选取2011年6月-2015年12月本院畲族哮喘儿童68例为试验组,同期选取健康畲族儿童68例为对照组。两组均进行FeNO浓度及肺功能指标检测,包括第1秒用力呼气容积占预计值百分比(FEV1%)、用力肺活量(FVC)、呼气峰流速占预计值百分比(PEF%),其中选用试验组治疗前及治疗3、6、9个月的FeNO浓度及肺功能指标,并分析FeNO浓度及FEV1%的相关性。结果:试验组治疗3、6、9个月的FeNO值均较治疗前显著降低,后逐渐缓慢下降(P<0.05);试验组治疗3、6、9个月的肺功能指标(FEV1%、PEF、FVC)较治疗前迅速升高,后逐渐缓慢升高(P<0.05);随访9个月时,试验组哮喘控制45例、部分控制15例,未控制8例;试验组完全控制、部分控制及未控制患儿的FeNO依次升高,FEV1%依次降低(P<0.05);相关性分析显示,FEV1%与FeNO呈负相关(r=-0.35,P<0.05);试验组治疗前、治疗3个月后的FeNO值均明显高于对照组(P<0.05);试验组治疗前、治疗3个月后的FEV1%值均低于对照组(P<0.05)。结论:FeNO在评估哮喘儿童的气道炎症方面是一项很好的指标,FeNO及肺功能间具有相关性,二者互为补充,共同应用可提高儿童哮喘控制及诊治水平。

【Abstract】 Objective:To compare the concentration of fractional exhaled nitric oxide(FeNO) with the index of lung function and evaluate the value of both of them in the diagnosis of bronchial asthma in children.Method:A total of 68 children with asthma in the She nationality of our hospital from June 2011 to December 2015 were selected as experimental group,and 68 children of the healthy She nationality were selected as control group in the same period.They were detected FeNO concentration and pulmonary function index,included 1 second forced expiratory volume accounted for the percentage of estimated value(FEV1%),forced vital capacity(FVC),peak expiratory flow percentage of predicted value(PEF%),the concentration of FeNO and pulmonary function of the experimental group before and after treatment 3,6 and 9 months were selected,and the correlation between the concentration of FeNO and FEV1% was analyzed.Result:After treatment 3,6 and 9 months,the FeNO values of in experimental group were significantly lower than those of before treatment,and then gradually decreased(P<0.05),the lung function index(FEV1%,PEF,FVC) of experimental group was higher than that of before treatment,and then gradually increased(P<0.05).After 9 months of follow-up,there were 45 cases of asthma control,15 cases of partial control,and 8 cases of not controlled in experimental group,the FeNO values of complete control,partial control and uncontrolled in experimental group children increased in turn,and the FEV1% decreased in turn(P<0.05).Correlation analysis showed that FEV1% was negatively correlated with FeNO(r=-0.35,P<0.05).The FeNO values of experimental group before treatment and after 3 months treatment were significantly higher than those of control group(P<0.05),the FEV1% of experimental group before treatment and after 3 months treatment were all lower than those of control group(P<0.05).Conclusion:FeNO is a good indicator for evaluating airway inflammation in children with asthma,and there is a correlation between FeNO and lung function,and the two complement each other,the common application can improve the control and treatment of asthma in children., 百拇医药(王晶晶 郑明平)
