地佐辛复合丙泊酚静脉全麻在四维超声子宫输卵管造影术中安全性和麻醉效果研究(1)
【摘要】 目的：研究地佐辛聯合丙泊酚静脉全麻在四维超声子宫输卵管造影术(hysterosalpingography，HSG)中的麻醉效果和安全性。方法：选取2016年5月-2017年5月在本院行四维超声HSG的患者80例，按照随机数字表法将其分为A组(地佐辛+丙泊酚)和B组(芬太尼+丙泊酚)，每组各40例。比较两组在麻醉前(T1)、静脉注药后2 min(T2)、插入造影管经子宫处注入空气堵住宫颈口(T3)、造影液体充满子宫以及双腔双侧输卵管(T4)及苏醒时(T5)的心率(HR)、舒张压(DBP)、血氧饱和度(SpO2)、收缩压(SBP)。观察患者术中及术后的不良反应，比较两组患者术中镇痛效果及苏醒后下腹痛程度(VAS评分)。结果：两组各个指标多时间点比较差异均有统计学意义(P<0.05)；且两组T2时的HR、DBP、SBP均较T1存在一过性下降(P<0.05)，但数值仍在正常范围内；A组患者镇痛优良率为80.00%，高于B组的70.00%，但比较差异无统计学意义( 字2=1.067，P=0.302)。结论：地佐辛复合靶控输注(TCI)丙泊酚应用于四维超声HSG安全可靠、镇痛效果佳、呼吸抑制发生率低，显著优于芬太尼，值得临床推广使用。

【Abstract】 Objective：To study the safety and anesthetic effect of Dezocine combined with Propofol intravenous anesthesia in four dimensional ultrasound hysterosalpingography(HSG).Method：A total of 80 patients with HSG of four dimensional ultrasound in our hospital from May 2016 to May 2017 were selected，according to the random number table method，they were divided into group A(Dezocine+Propofol) and group B(Fentanyl+Propofol)，40 cases in each group.The heart rate(HR)，diastolic pressure(DBP)，blood oxygen saturation(SpO2)，systolic pressure(SBP) at before anesthesia(T1)，2 min after intravenous injection(T2)，injection of air into the uterus and blocking the cervix of the cervix by inserting the contrast tube into the uterus(T3)，contrast liquid is full of uterus and double cavity bilateral fallopian tube(T4) and awakening(T5) between two groups were compared，the adverse effects of the patients during and after the operation were observed，and the analgesic effect and the lower abdominal pain after awakening(VAS score) between two groups were compared.Result：The multiple time points of each index in two groups were compared，the differences were statistically significant(P<0.05)，the HR，DBP and SBP of T2 in two groups were lower than those of T1(P<0.05)，but the value was still in the normal range.The good rate of analgesia in group A was 80.00%，which was higher than 70.00% in group B，but the difference was not statistically significant( 字2=1.067，P=0.302).Conclusion：Dezocine combined with target controlled infusion(TCI) Propofol in four dimensional ultrasound HSG is safe and reliable，the analgesic effect is good，the incidence of respiratory inhibition is low，which is significantly better than Fentanyl，worthy of clinical application.
