自体富血小板纤维蛋白凝胶治疗糖尿病足溃疡的效果研究(1)
【摘要】 目的：探究自體富血小板纤维蛋白凝胶治疗糖尿病足溃疡的效果。方法：将2016年
10月-2017年10月在本院接受治疗的60例糖尿病足溃疡患者作为研究对象，按照随机数字表法将患者分为对照组和观察组，各30例。对照组给予常规治疗，观察组在对照组基础上加用自体富血小板纤维蛋白凝胶治疗。对两组患者治疗前后创面感染恢复情况以及溃疡愈合时间、住院时间、费用、临床治疗效果进行综合评价。结果：观察组患者溃疡愈合时间、住院时间均短于对照组，住院费用低于对照组，差异均有统计学意义(t=6.893、5.698、12.045，P<0.05)。观察组患者治疗后7、14 d创面感染面积分别为(1.372±0.192)、(0.452±0.253)cm2，均明显少于治疗前的(2.583±0.503)cm2，差异均有统计学意义(P<0.05)；且与对照组治疗后7、14 d比较，差异均有统计学意义(t=13.486、16.397，P<0.05)。观察组总有效率为93.3%，高于对照组的70.0%，差异有统计学意义( 字2=9.382，P<0.05)。结论：采用自体富血小板纤维蛋白凝胶治疗糖尿病足溃疡，能够促进溃疡愈合，减少治疗费用，值得临床推广应用。

【Abstract】 Objective：To explore the effect of autologous platelet-rich fibrin gel on diabetic foot ulcer.Method：60 patients with diabetic foot ulcer treated in our hospital from October 2016 to October 2017 were treated as the study subjects，according to the random number table method，they were divided into the control group and observation group，30 cases in each group.The control group was treated with routine therapy，and the observation group was treated with autologous platelet-rich fibrin gel on the basis of the control group.The wound infection recovery before and after treatment，ulcer healing time，hospitalization time，cost，and clinical treatment effect of the two groups were evaluated comprehensively.Result：The healing time and hospitalization time of ulcer in the observation group were shorter than those in the control group，and the hospitalization cost was lower than that in the control group，the differences were statistically significant(t=6.893，5.698，12.045，P<0.05).On the 7th，14th day after treatment，the infection area of wound in the observation group were (1.372±0.192)，(0.452±0.253)cm2，which were significantly lower than(2.583±0.503)cm2 before treatment，the differences were statistically significant(P<0.05)；and compared with the control group on the 7th，14th day after treatment，the differences were statistically significant(t=13.486，16.397，P<0.05).The total effective rate of the observation group was 93.3%，which was higher than 70.0% of the control group，the difference was statistically significant( 字2=9.382，P<0.05).Conclusion：The treatment of diabetic foot ulcer with autologous platelet-rich fibrin gel can promote ulcer healing and reduce the cost of treatment，it is worth popularizing and applying in clinic.
