观察腹腔镜胆囊切除术治疗急性结石性胆囊炎合并糖尿病患者的效果(1)

     【摘要】 目的：觀察腹腔镜胆囊切除术治疗急性结石性胆囊炎合并糖尿病患者的效果。方法：选取本院2014年9月-2017年9月收治的90例急性结石性胆囊炎合并糖尿病患者，按抽签法分为观察组(n=45)和对照组(n=45)。对照组予以传统开腹手术治疗，观察组予以腹腔镜胆囊切除术治疗。比较两组患者手术情况、并发症发生率、血糖指标、生活质量、胰岛素用量、出院时间。结果：观察组的手术时间、排气时间、肠鸣音恢复时间、下床活动时间、切口长度、出院时间均明显短于对照组(P<0.05)；观察组的术中出血量、并发症、胰岛素用量均明显少于对照组(P<0.05)；观察组生活质量明显高于对照组(P<0.05)；观察组与对照组血糖指标比较，差异均无统计学意义(P>0.05)。结论：腹腔镜胆囊切除术可有效缩短急性结石性胆囊炎合并糖尿病患者的治疗时间，减少并发症，改善生活质量，治疗期间良好的控制血糖可保证手术效果，值得借鉴。

    【Abstract】 Objective：To observe the effect of laparoscopic cholecystectomy in the treatment of acute calculous cholecystitis complicated with diabetes mellitus.Method：From September 2014 to September 2017 90 patients with acute calculous cholecystitis complicated with diabetes mellitus were collected and divided into the observation group(n=45) and the control group(n=45).The control group was treated with traditional laparotomy and the observation group with laparoscopic cholecystectomy.The surgical conditions，the incidence of complications，blood glucose indicators，quality of life，insulin consumption，discharge time of two groups were compared.Result：The time of operation，the time of recovery of the bowel sounds，the time of getting out of bed，the length of incision and the length of discharge and the time of discharge of the observation group were significantly shorter than those of the control group(P<0.05).The intraoperative blood loss，complications and insulin dosage in the observation group were significantly less than those in the control group(P<0.05)，and the quality of life in the observation group was significantly higher than that in the control group，the differences were statistically significant(P<0.05).Comparison of blood glucose indexes between the observation group and the control group，the differences were not statistically significant(P>0.05).Conclusion：Laparoscopic cholecystectomy can effectively shorten the treatment time of acute calculous cholecystitis with diabetes mellitus，reduce the complication and improve the quality of life.Good blood glucose control during treatment can guarantee the effect of operation，it is worth of popularizing.
