甘精胰岛素联合阿卡波糖治疗老年初诊2型糖尿病患者24小时动态血糖变化观察(1)
【摘要】 目的：觀察探讨甘精胰岛素联合阿卡波糖治疗老年初诊2型糖尿病的临床疗效及血糖变化，总结临床应用价值。方法：选取本院收治的老年初诊2型糖尿病患者60例，按照随机数字表抽取法平均分为2组，各有30例，观察组给予甘精胰岛素联合阿卡波糖治疗，对照组单纯给予阿卡波糖治疗，观察对比两组患者24小时动态血糖变化、治疗前后胰岛功能及不良反应。结果：两组患者治疗3个月后的平均血糖、FBG、2 h PG、最高血糖、HbA1c与治疗前相比，均明显降低(P<0.05)；24 h血糖达标时间与治疗前相比，明显延长(P<0.05)；观察组治疗3个月后与对照组相比，改善水平更为理想(P<0.05)。两组患者治疗3个月后的Homa-β，Homa-IR与治疗前组内相比有明显差异(P<0.05)，组间比较也有明显差异(P<0.05)；两组患者不良反应发生率比较存在明显差异(P<0.05)。结论：甘精胰岛素联合阿卡波糖治疗老年初诊2型糖尿病的效果确切，有助改善胰岛功能，稳定血糖波动，减少低血糖事件发生，值得合理推广。

【Abstract】 Objective：To observe and probe into the clinical effect and the change of blood sugar of Insulin Glargine combined with Acarbose in treatment of elderly patients with type 2 diabetes，and to summarize the value of clinical application.Method：60 elderly patients with type 2 diabetes and who treated in our hospital were selected，they were average divided into two groups according to the random number table extraction，30 cases in each group.The observation group was given Insulin Glargine combined with acarbose treatment，while the control group was treated with acarbose treatment.Then，the 24 hours dynamic blood glucose change， islet function and adverse reaction before and after treatment of two groupswere observed and compared.Result：The average blood glucose， FBG，2 h PG， the highest blood sugar and HbA1c after the treatment for 3 months of two groups were compared with before the treatment，and which significantly lower (P<0.05).The 24 h blood glucose standard time was compared with before the treatment，and which was significantly longer(P<0.05).After the treatment for 3 months，the observation group was compared with the control group，and the level of improvement was more ideal(P<0.05).The Homa-β and Homa-IR of two groups after treatment for 3 months were significantly different from those before treatment (P<0.05)， and there was also significant difference between the two groups (P<0.05).There was significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusion：The effect of Insulin Glargine combined with Acarbose in treatment of elderly patients with newly diagnosed type 2 diabetes are exact，which is helpful to improve the function of islet， stabilize the fluctuation of blood sugar and reduce the occurrence of hypoglycemia，thus it is worth reasonable promotion., 百拇医药(孔令敏 汤海南 陈维东 李平)
