高频超声探查正常成人足底骨骼毗邻关系解剖结构、超声检查方法及声像图特征研究(1)
【摘要】 目的:研究高頻超声探查正常成人足底骨骼毗邻关系。方法:随机选择2016年7月-2017年12月至本院体检的正常健康成人60例作为研究对象,所有参与研究人员均给予足底超声检查。观察正常成人足底骨骼毗邻的肌肉、血管等解剖结构的毗邻关系,观察女性、男性跗内侧动脉、跗外侧动脉、腓动脉穿支、足底深动脉的收缩期峰值血流速度,以及左右两侧足底上述指标情况。结果:足底腱膜呈低回声,其起点附着在跟骨近端的趾面,趾短屈肌在超声显示为低回声的肌性结构,位于足底腱膜的深方,其浅方为足底垫,超声显示为大范围不均匀的较强回声。小趾展肌的纵断面能够见到小趾展肌的起点,为跟骨趾面外侧突的前方,而展肌纵断面能够见到展肌的起点为跟骨趾面的内侧突前方。男性与女性参与研究人员的跗内侧动脉、跗外侧动脉、腓动脉穿支和足底深动脉收缩期血流峰值比较,差异均无统计学意义(P>0.05);参与研究人员的左侧足底与右侧足底跗内侧动脉、跗外侧动脉、腓动脉穿支和足底深动脉收缩期血流峰值比较,差异均无统计学意义(P>0.05)。结论:高频超声能清晰显示正常成人足底骨骼毗邻关系,包括肌肉、血管、神经走形、附着点等,对于足底病变的诊断能够提供可靠的对照,为病理状态下的情况进行判断,从而为疾病的诊断和预后提供确切依据。

【Abstract】 Objective:To study the adjacent relationship of the plantar bones of normal adults by high frequency ultrasound.Method:A total of 60 healthy adults from July 2016 to December 2017 in our hospital were randomly selected as the subjects.All the participants were examined by plantar ultrasound.The adjacent relationship of muscle,blood vessels and other anatomical structures of the plantar bones of normal adults were observed,peak systolic velocity of medial tarsal artery,lateral tarsal artery,perforator of peroneal artery,deep plantar artery of female and male,and the left and right sides of the plantar were observed.Result:The plantar aponeurosis showed low echo,the starting point was attached to the toe surface at the proximal end of the calcaneus.flexor digitorum brevis in ultrasound showed hypoechoic muscular structure,located in the deep side of the plantar aponeurosis,and its shallow side was a plantar pad,ultrasound displayed a large range of uneven strong echo.The longitudinal section of the abductor digiti minimi muscular could see the starting point of the abductor digiti minimi muscular,which was the front of the lateral process of the calcaneal toe,and the longitudinal section of abductor hallucis muscle could see abductor hallucis muscle for the front of medial process of calcaneus toe.There were no significant differences in the peak systolic blood flow of the medial tarsal artery,lateral tarsal artery,perforating branch of the peroneal artery and the deep plantar artery between male and female participants(P>0.05).There were no significant differences in the peak systolic blood flow of the medial tarsal artery,lateral tarsal artery,perforating branch of the peroneal artery and the deep plantar artery between the left foot plantar and the right foot plantar(P>0.05).Conclusion:High frequency ultrasound can clearly display the adjacent relationship of normal adults plantar bones,including muscle,blood vessels,nerve shape,attachment point,etc.It can provide reliable control for the diagnosis of plantar lesions,and can be used to judge the condition of pathological state,so as to provide the exact basis for the diagnosis and prognosis of the disease., 百拇医药(张永红 鄂占森 周富强 施晓琳)
