无创血流动力学参数、左室射血分数联合BNP水平检测评估心力衰竭患者血流动力学的价值(1)
【摘要】 目的:分析无创血流动力学参数、左室射血分数联合BNP水平检测评估心力衰竭患者血流动力学的价值。方法:选取2013年6月-2017年6月本院收治的68例心力衰竭患者为研究对象,检测患者无创血流动力学参数、左室射血分数联合BNP水平,并分析心力衰竭患者的血流动力学评估价值。结果:随着心力衰竭患者血中BNP水平升高,其房缩波波幅(WA)、舒张功能指数(O/C)、肺动脉楔压(PCAP)、总外周阻力(TPR)水平均顯著升高(P<0.05);每搏做功(SW)、搏功指数(SWI)、每分做功(CW)、室缩波波幅(C)、心搏出量(SV)、心输出量(CO)、心脏指数(CI)及心缩力指数(HI)水平均显著降低(P<0.05)。BNP与WA、O/C、PCAP及TRP水平均呈显著正相关关系(P<0.05),与SW、SWI、CW、C、SV、CO、CI及HI水平均呈显著负相关关系(P<0.05)。随着LVEF水平升高,WA、O/C、TRP及PCAP均显著降低(P<0.05),CW、SW、SWI、SV、CO、CI、HI及C水平均显著升高(P<0.05);LVEF与患者WA、O/C、PCAP及TPR水平均呈显著负相关关系(P<0.05),与SW、SWI、CW、C、SV、CO、CI及HI水平均呈显著正相关关系(P<0.05)。结论:心力衰竭患者采用无创血流动力学参数、左室射血分数联合BNP水平检测后可有效评估患者血流动力学指标,可用于评估患者病情变化,具有较高的临床应用价值。

【Abstract】 Objective:To analyze noninvasive hemodynamic parameters,left ventricular ejection fraction (LVEF) combined with BNP level monitoring to evaluate hemodynamics in patients with heart failure.Method:68 patients with heart failure were selected in our hospital from June 2013 to June 2017 as the subjects.The noninvasive hemodynamic parameters,left ventricular ejection fraction and BNP levels were detected, and the hemodynamic evaluation value of heart failure patients was analyzed.Result:With the increase of serum BNP level in patients with heart failure,the levels of WA,O/C,PCAP,TPR were significantly higher(P<0.05),the levels of SW,SWI,CW,C,SV,CO,CI,HI were significantly decreased(P<0.05).There were significant positive correlations between BNP and WA,O/C,PCAP,TRP(P<0.05),there were negative correlations between BNP and SW,SWI,CW,C,SV,CO,CI and HI(P<0.05).With the increase of LVEF level,the levels of WA,O/C,TRP,PCAP were decreased significantly(P<0.05),the levels of CW,SW,SWI,SV,CO,CI,HI,C were significantly increased(P<0.05).There were significant negative correlations between LVEF and WA,O/C,PCAP,TPR(P<0.05),there were significant positive correlations between LVEF and SW,SWI,CW,C,SV,CO,CI,HI(P<0.05).Conclusion:Heart failure patients with noninvasive hemodynamic parameters and left ventricular ejection fraction combined with BNP level monitoring can effectively evaluate hemodynamic indexes,and can be used to assess the patient’s condition changes,so it has high clinical value.
