卡环固位式囊肿塞减压术治疗大型颌骨囊肿的临床疗效观察(1)
【摘要】 目的：探討卡环固位式囊肿塞减压术治疗大型颌骨囊肿的临床疗效。方法：选取2014年2月-2016年12月本院收治的40例大型颌骨囊肿患者，按照随机数字表法将其分为研究组和对照组，各20例。对照组采取刮除术治疗，研究组采取卡环固位式囊肿塞减压术治疗。比较两组术中出血量、手术时间和术后感染率；随访1年，比较两组并发症发生率和远期疗效。结果：研究组术中出血量和手术时间分别为(10.5±1.5)mL、(38.6±2.3)min，均少于对照组的(89.6±10.8)mL、(67.6±7.9)min，比较差异均有统计学意义(P<0.05)；研究组术后感染发生率为5.00%，显著低于对照组的35.00%(P<0.05)；研究组并发症发生率为20.00%，显著高于对照组的70.00%(P<0.05)，研究组囊腔缩小体积、骨密度水平、骨质增生厚度分别为(82.21±3.56)%、(0.48±0.12)g/cm2、(35.36±2.02)mm，均显著高于对照组的(36.24±3.24)%、(0.25±0.13)g/cm2、(18.32±3.98)mm，比较差异均有统计学意义(P<0.05)。结论：卡环固位式囊肿塞减压术治疗大型颌骨囊肿的效果显著，手术创口小，出血量少，术后感染及并发症发生率较低，远期疗效更佳，减少了对患者面部的损害，值得在临床上推广应用。

【Abstract】 Objective：To investigate the clinical efficacy of clasp fixation cyst plug decompression in treatment of large-scale jaw cyst.Method：A total of 40 patients with large-scale jaw cyst treated in our hospital from Fed 2014 to Dec 2016 were selected.According to the random number table method，they were divided into study group and control group，20 cases in each group.The control group was treated with curettage，and study group was treated with clasp fixation cyst plug decompression.The intraoperative blood loss，operation time and postoperative infection rate between two groups were compared，follow up for 1 year，the complication rate and long-term efficacy between two groups were compared.Result：The intraoperative blood loss and operation time in study group were(10.5±1.5) mL and(38.6±2.3) min，which were significantly less than(89.6±10.8) mL and(67.6±7.9) min in control group，the differences were statistically significant(P<0.05).The postoperative infection rate was 5.00% in study group，it was significantly lower than 35.00% in control group(P<0.05).The total complication rate in study group was 20.00%，significantly higher than 70.00% in control group(P<0.05).The cystic reduced volume，bone mineral density and bone hyperplasia thickness in study group were(82.21±3.56)%，(0.48±0.12)g/cm2，(35.36±2.02)mm，were higher than(36.24±3.24)%，(0.25±0.13)g/cm2，(18.32±3.98)mm in control group，the differences were statistically significant(P<0.05).Conclusion：The effect of clasp fixation cyst plug decompression in treatment of large-scale jaw cyst is remarkable，the surgical wound is small，the amount of bleeding is less，the incidence of postoperative infection and complications is low，and the long-term efficacy is better，reducing the damage to patient’s face.It is worthy of clinical application.
