CT对肝脏良性占位性病变及肝癌的鉴别诊断价值分析(1)
【摘要】 目的：探討CT对肝脏良性占位性病变及肝癌的鉴别诊断价值。方法：选取在本院2015年3月-2016年6月接受治疗的肝脏占位性病变患者90例作为研究对象，分为三组，A组患者均为原发性肝癌患者，B组均为肝转移癌患者，C组均为肝血管瘤患者，每组各30例。CT扫描使用的是飞利浦64排型的螺旋CT机，采用的扫描模式为全肝灌注模式。结果：三组患者在接受了CT灌注扫描之后，通过对三组患者的扫描结果进行对比发现，B组患者的肝动脉灌注量(HAP)在三组当中是最低的，而C组患者的HAP却是最高的。在门静脉灌注量(PVP)上，A组患者的PVP值最低，C组最高。且三组患者两两之间进行对比，差异均有统计学意义(P<0.05)。但是在总肝灌注量(TLP)这一指标上，A、B两组之间差异无统计学意义(P>0.05)，但是C组和这两组之间差异有统计学意义(P<0.05)。对三组患者病灶周围CT灌注参数进行了对比发现，在HAP和HPI指标上，B组与A、C两组之间均存在显著差异，但A、C两组之间差异无统计学意义(P>0.05)。结论：采用CT灌注扫描成像技术对患者进行检查可以对患者所患疾病种类进行有效的鉴别与诊断，具有较高的临床应用价值。

【Abstract】 Objective：To investigate the value of CT in the differential diagnosis of liver benign space occupying lesions and liver cancer.Method： The liver lesions of 90 patients in our hospital from March 2015 to June 2016 were selected as the research objects，they were divided into three groups，A group was primary liver cancer，B group was metastatic liver cancer，C group was hepatic hemangioma，30 patients in each group.CT scan used PHILPS 64 slice spiral CT，scanning mode type was the whole liver perfusion model.Result：After three groups of patients received CT perfusion scan， through comparing the scanning results of three groups of patients，it found that the hepatic artery perfusion volume (HAP) of B group was the lowest among the three groups，while the HAP of C group was the highest.In the portal vein perfusion (PVP)，the PVP of the patients in A group was the lowest，and C group was the highest，the differences were statistically significant(P<0.05).However，there was no statistically significant difference in the total hepatic perfusion volume (TLP) between A group and B group(P>0.05)，but the difference between C group and the other two groups was statistically significant(P<0.05).The CT perfusion parameters of the three groups of patients were compared，it found that there were significant differences on HAP and HPI between B group and A，C groups(P<0.05)，but there was no significant difference between A group and C group(P>0.05).Conclusion：CT perfusion imaging can effectively identify and diagnose the disease types of patients， and has high clinical application value.
