新诊断糖尿病患者甘油三酯/高密度脂蛋白胆固醇比值与胰岛素抵抗的相关性分析(1)

     【摘要】 目的:探讨新诊断糖尿病患者甘油三酯/高密度脂蛋白胆固醇比值(TG/HDL-C)与胰岛素抵抗的相关性。方法:选取2016年10月-2017年10月本院接诊的164例新诊断糖尿病患者为观察组,另选取同期在本院进行体检的150名健康志愿者为对照组。比较两组体重指数(BMI)、腰臀比(WHR)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白(HbA1c)、胰岛素(FINS)、TG/HDL-C、胰岛素抵抗指数(HOMA-IR)水平,分析各指标间的相关关系以及胰岛素抵抗的影响因素。结果:观察组BMI、WHR、FPG、TC、TG、LDL-C、TG/HDL-C、FINS、HbA1c、HOMA-IR水平均高于对照组,HDL-C水平低于对照组,比较差异均有统计学意义(P<0.05);相关性分析显示,TG/HDL-C与FPG、HOMA-IR、TG、TC均呈正相关(r=0.241、0.526、0.751、0.236,P<0.05),與HDL-C呈负相关(r=-0.227,P<0.05);HOMA-IR与BMI、FPG、TG、WHR均呈正相关(r=0.272、0.338、0.455、0.172,P<0.05),与HDL-C呈负相关(r=-0.236,P<0.05);多元回归分析结果显示,WHR、TG/HDL-C均为影响HOMA-IR的危险因素(P<0.05)。结论:新诊断糖尿病患者的胰岛素抵抗与TG/HDL-C有着密切联系,临床可将TG/HDL-C水平作为评估新诊断糖尿病患者胰岛素抵抗程度的有效指标。

    【Abstract】 Objective:To investigate the relationship between triglyceride/high-density lipoprotein cholesterol ratio(TG/HDL-C) and insulin resistance in newly diagnosed diabetic patients.Method:A total of 164 newly diagnosed diabetic patients in our hospital from October 2016 to October 2017 were selected as observation group,and 150 healthy volunteers who underwent physical examination in our hospital during the same period were selected as the control group.The body mass index(BMI),waist to hip ratio(WHR),fasting blood glucose(FPG),triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),glycated hemoglobin(HbA1c),insulin(FINS),TG/HDL-C,insulin resistance index(HOMA-IR) between two groups were compared,the correlation between the indexes and the influencing factors of insulin resistance were analyzed.Result:The levels of BMI,WHR,FPG,TC,TG,LDL-C,TG/HDL-C,FINS,HbA1c and HOMA-IR in observation group were higher than those of control group,while the levels of HDL-C were lower than those of control group,the differences were statistically significant(P<0.05).The correlation analysis showed that TG/HDL-C was positively correlated with FPG,HOMA-IR,TG and TC(r=0.241,0.526,0.751,0.236,P<0.05),and negatively correlated with HDL-C(r=-0.227,P<0.05).HOMA-IR was positively correlated with BMI,FPG,TG and WHR(r=0.272,0.338,0.455,0.172,P<0.05),and negatively correlated with HDL-C(r=-0.236,P<0.05).Multiple regression analysis showed that WHR and TG/HDL-C were risk factors for HOMA-IR(P<0.05).Conclusion:Insulin resistance in newly diagnosed diabetic patients is closely related to TG/HDL-C,and the TG/HDL-C level can be used as an effective index to evaluate the degree of insulin resistance in newly diagnosed diabetic patients., 百拇医药(孙燕 李婉媚)
