综合性护理干预在改善中老年肱骨近端骨折患者术后疼痛程度及减少术后并发症中的作用(1)
【摘要】 目的：研究分析综合性护理干预在改善中老年肱骨近端骨折患者术后疼痛程度及减少术后并发症中的作用。方法：选取2016年11月-2018年2月于本院骨科确诊为肱骨近端骨折拟行手术治疗的100例中老年患者作为研究对象，按照随机数字表法将其分为试验组和对照组，每组50例。对照组给予常规护理干预，试验组在对照组的基础上给予综合护理干预，观察比较两组干预前后的疼痛程度、肩关节功能以及护理满意度、术后并发症发生情况。结果：干预后1、2、3 d，试验组的VAS评分均明显低于对照组，差异均有统计学意义(P<0.05)。干预后3、6个月，试验组的肩关节Constant-Murley评分均明显低于对照组，差异均有统计学意义(P<0.05)。试验组术后并发症发生率为20.0%明显低于对照组的6.0%，差异有统计学意义(字2=5.400，P=0.020)。试验组护理总满意度明显高于对照组，差异有统计学意义(字2=4.882，P=0.027)。结论：综合性护理干预中老年肱骨近端骨折能够显著改善患者肩关节活动功能，降低术后疼痛程度，減少术后并发症，提高患者对护理满意度，值得临床推广应用。

【Abstract】 Objective：To study and analyze the role of comprehensive nursing intervention in improving postoperative pain degree and reducing postoperative complications in middle-aged and elderly patients with proximal humeral fractures.Method：A total of 100 middle-aged and elderly patients with proximal humerus fracture diagnosed in orthopaedic department of our hospital who undergone minimally invasive surgery from November 2016 to February 2018 were selected as the study objects.According to the random number table method，they were divided into experimental group and control group，50 cases in each group.The control group was given routine nursing intervention，the experimental group was given comprehensive nursing intervention on the basis of the control group.The degree of pain，shoulder joint function before and after intervention，nursing satisfaction and postoperative complications between the two groups were observed and compared.Result：After intervention 1，2 and 3 days，the VAS scores of the experimental group were significantly lower than those of the control group，the differences were statistically significant(P<0.05).After intervention 3 and 6 months，the Constant-Murley scores of shoulder joint in the experimental group were significantly lower than those in the control group，the differences were statistically significant(P<0.05).The incidence of postoperative complications in the experimental group was 20.0%，which was significantly lower than 6.0% in the control group，the difference was statistically significant(字2=5.400，P=0.020).The total nursing satisfaction degree of the experimental group was significantly higher than that of the control group，the difference was statistically significant(字2=4.882，P=0.027).Conclusion：Comprehensive nursing intervention for middle-aged and elderly patients with proximal humeral fractures can significantly improve the shoulder joint function，reduce postoperative pain degree and postoperative complications，improve patients’ satisfaction with nursing，which is worthy of clinical application., http://www.100md.com(吕燕碧)
