Focus超声刀和传统电刀在甲状腺手术中对甲状旁腺功能及切口炎性反应的影响(1)
【摘要】 目的：探討在甲状腺手术中应用Focus超声刀对甲状旁腺功能及切口炎性反应的影响，并与传统电刀进行比较。方法：选取进行甲状腺手术的249例甲状腺占位患者为研究对象，根据方法不同将其分为Focus组(n=148)和对照组(n=101)。Focus组采用Focus超声刀进行手术，对照组采用传统电刀进行手术，比较两组患者手术情况(手术时间、术中出血量、术后24 h疼痛评分、术后引流量、拔管时间)、甲状旁腺激素(PTH)变化、切口炎性反应、术后并发症发生率和随访1年内复发率。结果：Focus组手术情况优于对照组(P<0.01);Focus组手术前后PTH水平比较，差异无统计学意义(P>0.05)，对照组术后24 h及术后1、4周PTH水平均低于手术前(P<0.01);两组手术前、手术中PTH水平比较，差异均无统计学意义(P>0.05)，但Focus组术后24 h及术后1、4周PTH水平均高于对照组(P<0.05);Focus组术后24 h及术后第3、5、7天的切口炎性反应程度均优于对照组(P<0.01);Focus组并发症发生率低于对照组(P<0.01)，但两组术后1年内的复发率比较，差异无统计学意义(P>0.05)。结论：Focus超声刀应用于甲状腺手术能明显改善患者的手术情况，减轻手术后甲状旁腺功能的影响、切口的炎性反应和并发症发生率。

【Abstract】 Objective：To investigate the effect of Focus ultrasound scalpel on parathyroid function and incision inflammatory response in thyroid surgery，and compare it with traditional electric scalpel.Method：A total of 249 patients with thyroid mass underwent thyroid surgery were selected.According to different methods，they were divided into Focus group(n=148)and control group(n=101).Focus group was operated with Focus ultrasound scalpel，while control group was operated with traditional electric scalpel.The surgical conditions(operation time，bleeding volume，pain score at 24 hours，drainage volume and extubation time)，changes of parathyroid hormone(PTH)，inflammatory reaction of incision，incidence of complications and recurrence rate within 1 year between two groups were compared.Result：The operation condition of Focus group was better than that of control group(P<0.01).The PTH levels before and after operation in Focus group was compared，the difference was not statistically significant(P>0.05)，after operation 24 hours，1 and 4 weeks，PTH level in control group were lower than those of before operation(P<0.01)，the PTH levels before and during operation between two groups was compared，the difference was not statistically significant(P>0.05)，but the PTH levels in Focus group were higher than those of control group at 24 hours，1 and 4 weeks after operation(P<0.05).The inflammatory reaction of incision in Focus group were better than those of control group after operation 24 hours，3，5 and 7 days(P<0.01).The incidence of complications in Focus group was lower than that of control group(P<0.01).However，the recurrence rate within 1 year after operation was compared between two groups，the difference was not statistically significant(P>0.05).Conclusion：Focus ultrasound scalpel used in thyroid surgery can significantly improve the surgical condition of patients，reduce the function of parathyroid gland after surgery，inflammatory reaction of incision and incidence of complications., 百拇医药(罗金现 温建燔 高训锋)
