Ⅱ型糖尿病患者泪膜稳定性相关因素分析(1)
【摘要】 目的：探討各种相关因素对Ⅱ型糖尿病患者泪膜稳定性的影响。方法：选取2016年1月-2017年6月本院确诊为Ⅱ型糖尿病患者120例(240眼)为糖尿病组，同期检查的无糖尿病患者120例(240眼)为对照组。研究对象均进行干眼症评分、泪膜厚度、泪液分泌情况及泪膜破裂时间(BUT)等泪膜稳定性检查。比较两组泪膜稳定性情况以及糖尿病组不同年龄、性别、病程的泪膜稳定性情况，分析影响泪膜稳定性的因素。结果：糖尿病组干眼症评分低于对照组，泪膜厚度、泪液分泌情况、BUT均少于对照组，差异均有统计学意义(P<0.05);糖尿病组女性干眼症比例多于男性，泪膜厚度薄于男性，差异均有统计学意义(P<0.05);糖尿病组不同年龄段组的干眼症评分、泪膜厚度、泪液分泌情况及BUT比较，差异无统计学意义(P>0.05);病程<10年组的泪膜厚度、泪液分泌情况及BUT均优于病程≥10年组(P<0.05)，但不同病程者的干眼症评分比较差异无统计学意义(P>0.05)。Logistic回归分析显示，性别、病程均是影响糖尿病者泪膜稳定性的影响因素(P<0.05)。结论：糖尿病患者易发生泪液分泌及泪膜功能异常，女性且病程越长糖尿病患者泪膜稳定性越差。

【Abstract】 Objective：To explore the influence of various related factors on tear film stability in patients with type 2 diabetes mellitus.Method：A total of 120 patients(240 eyes)with type 2 diabetes diagnosed in our hospital from January 2016 to June 2017 were selected as diabetes group，120 patients(240 eyes)without diabetes were selected as control group.The subjects were examined for tear film stability including dry eye score，tear film thickness，tear secretion and tear film rupture time(BUT).The tear film stability of two groups was compared，and the tear film stability of the diabetic group in different age，sex and course of disease were compared，the factors affecting tear film stability were analyzed.Result：The dry eye score of diabetic group was lower than that of control group，the tear film thickness，tear secretion and BUT of diabetic group were lower than those of control group，the differences were statistically significant(P<0.05).The proportion of dry eye in female diabetes mellitus patients was higher than that of male patients，the thickness of tear film was thinner than that of male patients，the differences were statistically significant(P<0.05).The dry eye score，tear film thickness，tear secretion and BUT of different age groups in diabetes mellitus group were compared，the differences were not statistically significant(P>0.05).The tear film thickness，tear secretion and BUT of patients with course less than 10 years were better than those with course more than 10 years(P<0.05)，but the dry eye scores of patients with different course of disease were compared，the differences was not statistically significant(P>0.05).Logistic regression analysis showed that gender and course of disease were the influencing factors of tear film stability in diabetic patients(P<0.05).Conclusion：Diabetes patients are prone to tear secretion and tear film dysfunction，the female，the longer the course of diabetes，the worse the tear film stability., http://www.100md.com(蒋兆荣 董超 陈冰)
