低温等离子射频消融术配合鼻内镜手术对慢性鼻窦炎患者嗅觉功能及鼻腔通气状态的影响(1)

     【摘要】 目的:觀察低温等离子射频消融术配合鼻内镜手术对慢性鼻窦炎(chronic rhinosinusitis,CRS)患者嗅觉功能及鼻腔通气状态的影响。方法:选取2017年6月-2018年5月在本院治疗的CRS患者104例,应用随机数字表法分为对照组和观察组,各52例。对照组给予鼻内镜手术治疗,观察组接受低温等离子刀、鼻内镜手术治疗。比较两组手术后嗅觉功能、鼻腔通气功能(鼻腔容积、鼻阻力)、鼻腔黏膜纤毛清除功能及Lund-kennedy量表评分。结果:术前,两组嗅觉功能分级、鼻腔容积、鼻阻力、糖精清除时间(SCT)、Lund-kennedy评分对比,差异均无统计学意义(P>0.05)。术后,观察组嗅觉功能Ⅰ级患者占比63.46%,高于对照组的38.46%;鼻腔容积为(13.11±2.09)cm3,大于对照组的(11.05±1.57)cm3;SCT为(16.10±1.53)min低于对照组的(17.69±1.84)min;鼻阻力为(0.35±0.09)kPa/(L·s)低于对照组的(0.43±0.14)kPa/(L·s);Lund-kennedy评分为(4.23±0.85)分,低于对照组的(9.47±1.12)分,差异均有统计学意义(P<0.05)。结论:CRS患者接受低温等离子刀配合鼻内镜手术治疗有利于缓解患者临床症状,改善嗅觉功能及鼻腔通气功能。

    [Abstract] Objective: To observe the effects of low temperature plasma radiofrequency ablation combined with endoscopic sinus surgery on olfactory function and nasal ventilation in patients with chronic sinusitis (CRS). Method: A total of 104 patients with CRS who were treated in our hospital from June 2017 to May 2018 were selected, the patients were divided into control group and observation group by random number table method, 52 cases in each group. The control group received endoscopic sinus surgery, and the observation group received low temperature plasma knife and endoscopic sinus surgery. The olfactory function, nasal ventilation function (nasal cavity volume, nasal resistance), nasal mucociliary clearance function and Lund-kennedy scale score were compared between the two groups. Result: Before the surgery, there were no significant differences in olfactory function, nasal cavity volume, nasal resistance, saccharin clearance time (SCT) and Lund-kennedy score between the two groups(P>0.05). After the operation, the proportion of patients with olfactory function grade Ⅰ in the observation group was 63.46%, higher than 38.46% in the control group; and the nasal cavity volume was (13.11±2.09) cm3, higher than (11.05±1.57) cm3 in the control group; SCT was (16.10±1.53) minutes lower than (17.69±1.84) minutes of the control group; the nasal resistance was (0.35±0.09) kPa/(L·s) lower than (0.43±0.14) kPa/(L·s) of the control group; Lund-kennedy score was (4.23±0.85) scores, lower than (9.47±1.12) scores of the control group, the differences were statistically significant (P<0.05). Conclusion: CRS patients receiving low temperature plasma knife combined with endoscopic sinus surgery are helpful to alleviate clinical symptoms, improve olfactory function and nasal ventilation function.
