胎儿室间隔缺损筛查中断层超声显像技术的应用及其准确性研究(1)

     【摘要】 目的:探討胎儿室间隔缺损筛查中断层超声显像技术的应用及其准确性。方法:选取2017年5月-2019年7月于本院行产前超声筛查中疑似先天性心脏病的胎儿100例,分别进行一次二维超声心动图检查和超声断层显像技术检查,比较两种检查方式诊断室间隔缺损、两种方式室间隔缺损符合率、室间隔缺损图像。结果:断层超声显像技术诊断室间隔缺损的检出率高于二维超声心动图检查,漏诊率低于二维超声心动图检查,差异均有统计学意义(P<0.05)。超断层声显像技术检查室间隔缺损直径≤4 mm及直径>4 mm的符合率均高于二维超声心动图的符合率,差异均有统计学意义(P<0.05)。二维超声检查心动图室间隔缺损敏感性为86.9%,断层超声显像技术检查胎儿室间隔缺损敏感性为96.7%。结论:胎儿室间隔缺损使用断层超声显像技术的准确性较高,能够更加直观的筛选胎儿室间隔缺损,尤其在小型室间隔缺损、肌部室间隔缺损诊断具有较高的优势,值得推广应用。

    [Abstract] Objective: to investigate the application and accuracy of ultrasonography in the screening of fetal ventricular septal defect. Method: A total of 100 fetuses suspected of congenital heart disease during prenatal ultrasound screening in our hospital from May 2017 to July 2019 were selected. Two dimensional echocardiography and ultrasonic tomography were performed respectively. The two methods were compared for diagnosing ventricular septal defect, the coincidence rate of ventricular septal defect and the image of ventricular septal defect. Result: The detection rate of ventricular septal defect was higher than that of two-dimensional echocardiography, the rate of missed diagnosis is lower than that of two-dimensional echocardiography, the differences were statistically significant (P<0.05).     >4 mm by super-fault acoustic imaging was higher than that of two-dimensional echocardiography, the differences were statistically significant (P<0.05). The sensitivity of two-dimensional echocardiography to ventricular septal defect was 86.9%, the sensitivity of fetal ventricular septal defect was 96.7%. Conclusion: The accuracy of fetal ventricular septal defect is relatively high, it can screen fetal ventricular septal defect more intuitively, especially in the diagnosis of small ventricular septal defect and muscular ventricular septal defect has a higher advantage, which is worthy of popularization and application.
