腹腔镜根治性右半结肠切除术对患者免疫功能及胃肠激素的影响(1)
【摘要】 目的:探討腹腔镜根治性右半结肠切除术对患者免疫功能、胃肠激素与预后的影响。方法:将2018年3月-2019年10月本院收治的80例结直肠癌患者按照随机数字表法分为对照组与观察组,每组40例。对照组行传统开腹结直肠癌手术进行治疗,观察组给予腹腔镜根治性右半结肠切除术,观察比较两组手术相关情况、细胞免疫功能、胃肠激素及术后并发症等。结果:观察组术中出血量少于对照组,排便时间、肛门排气时间、住院时间均明显短于对照组(P<0.05);观察组术后免疫细胞T淋巴细胞CD3+、CD4+、CD8+、CD4+/CD8+减少水平均小于对照组(P<0.05);观察组胃动素(MTL)和胃泌素(GAS)减少水平均小于对照组(P<0.05);观察组术后并发症发生率低于对照组(12.5% vs 37.5%),差异有统计学意义(P<0.05)。结论:腹腔镜根治性右半结肠切除术对患者机体免疫功能及胃肠激素的影响均较小,且术后并发症发生率降低,安全有效,有助于促进患者术后康复。

[Abstract] Objective: To investigate the effect of laparoscopic radical right hemicolectomy on patients’ immune function, gastrointestinal hormones and prognosis. Method: Eighty patients with colorectal cancer treated in our hospital from March 2018 to October 2019 were divided into the control group and the observation group according to the random number table method, 40 cases in each group. The control group was treated with traditional open colorectal cancer surgery, the observation group was given laparoscopic radical right hemicolectomy. The surgical conditions, cellular immune function, gastrointestinal hormones and postoperative complications of two groups were observed and compared. Result: The amount of bleeding in the observation group was less than that in the control group, and the defecation time, anal exhaust time, and hospital stay were significantly shorter than those in the control group (P<0.05). The immune cell T lymphocytes CD3+, CD4+, CD8+, the levels of CD4+/CD8+ reduction of the observation group after surgery were smaller than those of the control group (P<0.05). The reduction levels of motilin (MTL) and gastrin (GAS) in the observation group were significantly smaller than those in the control group (P<0.05). The incidence of postoperative complications in the observation group compared with the control group (12.5% vs 37.5%), the difference was statistically significant (P<0.05). Conclusion: Laparoscopic radical right hemicolectomy has little effect on the patient’s immune function and gastrointestinal hormones, and the incidence of postoperative complications is reduced. It is safe and effective and can help promote postoperative recovery.
