群组管理对心房颤动RFCA术后患者的心理韧性及用药依从性的影响(1)
【摘要】 目的：探討群组管理对心房颤动RFCA术后患者的心理韧性及用药依从性的影响。方法：选择2018年1-12月于本院心内科收治的120例心房颤动RFCA术后的患者为研究对象。根据病区进行分组，一病区的60例作为观察组，二病区的60例作为对照组。观察组采用群组管理模式，对照组采用常规护理模式，比较两组患者的心理及用药依从性情况。结果：出院时及出院后3个月，观察组SAS及SDS评分均较入院时下降，且均低于对照组，差异均有统计学意义(P<0.05)。出院后3个月，观察组患者心理韧性评分较入院时升高，且高于对照组，差异均有统计学意义(P<0.05)。观察组患者的用药依从性优于对照组，差异有统计学意义(P<0.05)。出院后14 d、1个月、3个月，观察组INR达标率均高于对照组，差异均有统计学意义(P<0.05)。结论：群组管理对心房颤动RFCA术后出院患者自我管理效果较好，一定程度上改善患者的心理韧性，提高用药依从性，值得进一步推广。

[Abstract] Objective： To explore the effect of group management on psychological resilience and medication compliance of patients with atrial fibrillation after RFCA. Method： A total of 120 patients with ATRIAL fibrillation after RFCA surgery admitted to the department of Cardiology of our hospital from January to December 2018 were selected as study subjects. According to the ward， 60 cases in the first ward were taken as the observation group and 60 cases in the second ward as the control group. The observation group adopted group management mode， while the control group adopted routine nursing mode. The psychological status and medication compliance of the two groups were compared. Result： At discharge and three months after discharge， the SAS and SDS scores of the observation group were lower than those at admission， and lower than those of the control group， the differences were statistically significant (P<0.05). Three months after discharge， the score of resilience in the observation group was higher than that at admission， and higher than that in the control group， the differences were statistically significant (P<0.05). The medication compliance of the observation group was better than that of the control group， the difference was statistically significant (P<0.05). 14 days， 1 month and 3 months after discharge， the INR compliance rate of the observation group were higher than those of the control group， the differences were statistically significant (P<0.05). Conclusion： Group management has a good effect on self-management of discharged patients with atrial fibrillation after RFCA. To a certain extent， it can improve the psychological resilience and medication compliance of patients， which is worthy of further promotion.
