早产儿血清甲状腺激素浓度变化与疾病危重相关性的研究(1)
【摘要】 目的:探討早产儿血清甲状腺激素浓度变化与疾病危重的相关性。方法:选取2018年3月-2019年6月本院收治的早产儿847例,均行甲状腺激素检测,将诊断为低T3综合征者纳入研究,并按胎龄分为28~32周组、32+1~34周组、34+1~36周组,不同胎龄组间再根据其病情危重程度分为极危重组、危重组、非危重组。观察847例早产儿低T3综合征的发生情况,比较不同胎龄组的血清甲状腺激素水平及各胎龄组中极危重组、危重组、非危重组血清甲状腺激素水平,分析血清甲状腺激素水平与疾病严重程度的相关性。结果:847例早产儿发生低T3综合征82例(9.68%)。不同胎龄组三碘甲状腺原氨酸(T3)、甲状腺素(T4)水平比较,差异均有统计学意义(P<0.05);不同胎龄组促甲状腺激素(TSH)比较,差异无统计学意义(P>0.05)。各胎龄组中极危重组、危重组、非危重组血清T3、T4水平比较,差异均有统计学意义(P<0.05)。通过Spearman秩相关分析发现,血清T3、T4水平和疾病严重程度均呈负相关(P<0.05)。结论:早产儿血清甲状腺激素浓度变化与疾病危重程度密切相关,其血清T3和T4水平越低,病情越危重。

[Abstract] Objective: To investigate the correlation between changes in serum thyroid hormone concentration and critical illness in premature infants. Method: A total of 847 premature infants admitted to our hospital from March 2018 to June 2019 were selected and tested for thyroid hormone, patients diagnosed with low T3 syndrome were included in the study and divided into groups of 28 to 32 weeks, 32+1 to 34 weeks, and 34+1 to 36 weeks according to gestational age. According to the severity of the disease, the patients in different gestational age groups were divided into extremely critical group, critical group and non-critical group. The incidence of low T3 syndrome in 847 premature infants was observed, the serum thyroid hormone levels in different gestational age groups were compared, and the serum thyroid hormone levels of extremely dangerous recombinant, dangerous recombinant and non dangerous recombinant in each gestational age group were compared, the correlation between serum thyroid hormone level and disease severity was analyzed. Result: Low T3 syndrome occurred in 82 (9.68%) of 847 premature infants. Comparison of triiodothyronine (T3) and thyroxine (T4) levels in different gestational age groups, the differences were statistically significant (P<0.05). Comparison of thyroid stimulating hormone (TSH) in different gestational age groups, the difference was not statistically significant (P>0.05). The comparison of serum T3 and T4 levels in the medium, critical, and non-critical groups of different gestational age groups, the differences were statistically significant (P<0.05). Spearman rank correlation analysis showed that serum T3 and T4 levels were negatively correlated with disease severity (P<0.05). Conclusion: The change of serum thyroid hormone concentration in premature infants is closely related to the severity of the disease, the lower the serum T3 and T4 levels, the more critical the condition is., http://www.100md.com(王巧芳 晏长红 杨利)
