    【摘要】 目的：探討腹腔镜辅助左半结肠切除术中不保留肠系膜下动脉(IMA)的效果和安全性。方法：按照术中是否保留IMA将贵溪市人民医院2019年1月-2020年5月收治的101例行腹腔镜辅助左半结肠切除术患者分为对照组(n=50)和观察组(n=51)。对照组行术中保留IMA，观察组行IMA切除术。观察两组手术情况、术后恢复情况、复发情况以及并发症发生情况。结果：观察组手术时间短于对照组，切除肠管长度长于对照组，术中出血量少于对照组，淋巴清扫数多于对照组，差异均有统计学意义(P<0.05);观察组排便时间、排气时间、拔除引流管时间以及住院时间与对照组对比，差异均无统计学意义(P>0.05);观察组复发率(5.88%)与对照组(10.00%)对比，差异无统计学意义(P>0.05);观察组并发症发生率(7.84%)与对照组(12.00%)对比，差异无统计学意义(P>0.05)。结论：腹腔镜辅助左半结肠切除术中不保留IMA可通过简化手术步骤，避免术中意外损伤，减少出血，缩短手术时间，且不增加并发症发生率以及复发率，具有临床应用价值。
