    【摘要】 从延续性照护模式的概念与内涵、延续性照护在脑卒中患者中的研究现状、延续性照护效果评价指标对延续性照护模式在脑卒中患者中的相关研究进行综述。提出了现阶段研究的不足和未来的研究方向，以期为临床照护提供参考，为今后构建统一、有效的延续性照护模式开拓思路。

    [Abstract] This paper reviews the concept and connotation of continuous care model， the research status of continuous care in stroke patients， and the evaluation indicators of continuous care effect. This paper puts forward the shortcomings of the current research and the future research direction， in order to provide reference for clinical care and open up ideas for the construction of a unified and effective continuous care model in the future.
