【摘要】目的探讨城乡结合社区卫生服务中心对精神病康复管理的有效模式。方法社区精防领导小组联合社区卫生服务中心建立村卫生干部，患者家属(监护人)互通信息机制，通过开设“阳光心园”康复基地，健康教育，送药体检，分类、分级管理，群防群治，随访监控做好精神病患者出院后的康复管理服务。结果通过五年康复管理的探索，六灶社区的精神病患者有效监护率98.78%，显好率85.43%，肇事肇祸率﹤0.1%，社会参与率88.26%。管理有显著意义。结论社区卫生服务中心对于城乡结合社区精神病的康复管理模式有效可行，有效控制精神疾病病情，恢复社会功能和防止复发，精神病肇事肇祸率降低。
