经阴道彩超对输卵管间质部妊娠定位诊断的价值(1)
【摘要】 目的：探讨经阴道彩色多普勒超声(TVCDU)应用于输卵管间质部妊娠定位诊断中的价值。方法：回顾性分析31例患者的病历资料，均因临床疑诊输卵管间质部妊娠而住院手术治疗。术前均序贯予以经腹部彩色多普勒超声(TACDU)和TVCDU检查。以手术-病检结果作为诊断的金标准，对两种检查途径诊断输卵管间质部妊娠的敏感性、特异性及准确性进行比较分析。结果：31例患者中24例输卵管间质部妊娠，7例为宫角妊娠，均经手术-病理检查结果所确诊。TVCDU诊断间质部妊娠的敏感性(95.83%)、特异性(71.43%)和准确性(90.32%)高于TACDU诊断的79.17%、28.57%、67.74%，差异均有统计学意义(P<0.05)。结论：TVCDU应用于诊断输卵管间质部妊娠，相较于TACDU，可显著提高诊断的敏感性、特异性与准确性，可作为输卵管间质部妊娠筛查的首选方法。

【Abstract】 Objective：To explore the value of transvaginal color Doppler ultrasound(TVCDU) in the localization diagnosis of interstitial tubal pregnancy.Method：A retrospective analysis was made of the medical records of 31 patients，all of whom were hospitalized for surgical treatment due to clinically suspected tubal interstitial pregnancy.transabdominal color Doppler ultrasound(TACDU) and TVCDU were sequentially examined before operation.The sensitivity，specificity and accuracy of the two methods for diagnosis of interstitial tubal pregnancy were compared and analyzed with the results of surgery and pathology as the gold standard for diagnosis.Result：A total of 31 patients，including 24 cases of tubal interstitial pregnancy，7 cases of uterine horn pregnancy，were diagnosed by surgical-pathological examination.The sensitivity(95.83%)，specificity(71.43%) and accuracy(90.32%) of TVCDU in the diagnosis of interstitial pregnancy were higher than 79.17%，28.57%，67.74% of TACDU respectively with statistical significance(P<0.05).Conclusion：Compared with TACDU，TVCDU can significantly improve the sensitivity，specificity and accuracy of diagnosis，and can be used as the first choice for tubal interstitial pregnancy screening.

輸卵管间质部妊娠较为少见，发生率约占异位妊娠的4.20%。间质部妊娠一旦发生破裂，可于短时间内发生致命性腹腔大出血，严重威胁孕妇生命安全，产前明确诊断并及时予以治疗对改善患者预后具极其重要的临床价值[1]。近年来，随超声技术的不断发展，彩色多普勒超声已成为输卵管间质部妊娠定位诊断的最重要检查方法[2]。单纯的腹部彩色多普勒超声(transabdominal color doppler ultrasonography，TACDU)用于输卵管间质部妊娠的定位诊断，因影响因素较多，准确性受到一定的限制。本文回顾性分析2016年1月-2018年12月收治的31例临床疑诊输卵管间质部妊娠患者的影像和临床资料，旨探讨经阴道彩色多普勒超声(transvaginal color doppler ultrasonography，TVCDU)在输卵管间质部妊娠定位诊断中的价值。, 百拇医药(高红 王茜)
