特发性膜性肾病与IgA肾病的临床比较(1)

     【摘要】 目的：探討特发性膜性肾病(idiopathic membranous nephropathy，IMN)与IgA肾病的临床特点。方法：选择245例特发性膜性肾病患者(IMN组)及98例IgA肾病患者(IgA组)作为研究对象，所有病例均常规检测24 h尿蛋白、血清白蛋白、总胆固醇(CH)、低密度胆固醇(LDL)、甘油三酯(TG)和高密度胆固醇(HDL)，并将两组年龄、尿蛋白及血脂进行对比。结果：IMN组男性比例(60.8%)高于IgA组(45.9%)(P<0.05)。两组高血压发病率比较，差异无统计学意义(P>0.05)。IMN组年龄、24 h尿蛋白、CH、LDL、TG和HDL均高于IgA组(P<0.01)。IMN组血清白蛋白低于IgA组(P<0.05)。IMN组男性、女性、合并高血压及未合并高血压患者年龄、24 h尿蛋白、CH、HDL、LDL及TG均高于IgA组(P<0.05)。结论：IMN中高血压发病率与IgA肾病相近，IMN男性比例高于IgA肾病，IMN患者年龄、24 h尿蛋白及多项血脂指标均高于IgA肾病，IMN男性、女性、合并高血压及未合并高血压患者年龄、24 h尿蛋白、CH、HDL、LDL及TG均高于IgA肾病，通过对比研究可为临床诊治IMN及IgA肾病提供参考依据。

    【Abstract】 Objective：To investigate the clinical characteristics of idiopathic membranous nephropathy and IgA nephropathy.Method：A total of 245 patients with idiopathic membranous nephropathy(IMN group) and 98 patients with IgA nephropathy(IgA group) were selected as subjects of study.24 hour urinary protein，serum albumin，total cholesterol(CH)，low density lipoprotein cholesterol(LDL)，triglyceride(TG) and high density lipoprotein cholesterol(HDL) were routinely measured in all cases，and the age，urinary protein and blood lipid of the two groups were compared.Result：The proportion of male patients in the IMN group(60.8%) was higher than that of the IgA group(45.9%)(P<0.05).The incidence of hypertension was compared between the two groups，and the difference was not statistically significant(P>0.05).The age，24 hour urinary protein，CH，LDL，TG and HDL in the IMN group were higher than those of the IgA group(P<0.01)，while the serum albumin in the IMN group was lower than that of the IgA group(P<0.05).The age，24 hour urinary protein，CH，TG，HDL and LDL of men，women，patients with hypertension and without hypertension in the IMN group were all higher than those of the IgA group(P<0.05).Conclusion：The incidence of hypertension in IMN is close to IgA nephropathy.IMN has a higher proportion of men than IgA nephropathy.The age，24 hour urinary protein and multiple lipid indexes of IMN patients are all higher than those of IgA nephropathy.And the age，24 hour urinary protein，CH，TG，HDL and LDL of men，women，patients with hypertension and without hypertension of IMN are all higher than those of IgA nephropathy.Through comparative study can provide reference for clinical diagnosis and treatment of IMN and IgA nephropathy., http://www.100md.com(潘庆登 叶祖兰 宁万如)
