穴位按摩联合音乐疗法对股骨头坏死患者心理状态及生活质量的影响(1)
【摘要】 目的：探究穴位按摩联合音乐疗法对股骨头坏死患者心理状态及生活质量的影响。方法：将笔者所在医院骨科收治的52例股骨头坏死患者随机分为对照组与干预组，各26例。对照组给予常规护理，在此基础上，干预组给予穴位按摩联合音乐疗法护理，观察两组护理前后心理状态及生活质量水平。结果：护理后，两组焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均较护理前显著降低，且干预组降低幅度更显著(P<0.05);出院1个月后，干预组健康状况调查量表(SF-36)中生理功能、生理职能、躯体疼痛、总体健康、活力、社会功能、情感职能、精神健康评分均显著高于对照组(P<0.05)。结论：在股骨头坏死患者护理中应用穴位按摩联合音乐疗法，能够改善患者负性心理状态，提高生活质量。

[Abstract] Objective： To explore the effects of acupoint massage combined with music therapy on psychological states and quality of life of patients with femoral head necrosis. Method： A total of 52 patients with femoral head necrosis admitted to department of orthopedics of our hospital were randomly divided into the control group and the intervention group， 26 cases in each group. The control group was given routine nursing， and the intervention group was given acupoint massage combined with music therapy on this basis. Psychological states and quality of life before and after nursing were observed in the two groups. Result： After nursing， the scores of self-rating anxiety scale (SAS) and self-rating depression scale (SDS) in the two groups were significantly decreased compared with those before intervention， and the reduction were more significant in the intervention group (P<0.05). At 1 month after discharge， the scores of physical function， role physical， physical pain， general health， vitality， social function， emotional function and mental health of short-form health survey (SF-36) in the intervention group were significantly higher than those of the control group (P<0.05). Conclusion： Acupoint massage combined with music therapy in the nursing of patients with femoral head necrosis can improve the negative psychological states and enhance the quality of life.

股骨头坏死为骨科常见难治性疾病，具有髋关节疼痛、关节僵硬、功能障碍等临床表现，病程较长，治疗效果缓慢，患者极易产生沉重的心理负担而出现心理问题[1]。临床主要以髋关节置换术治疗股骨头坏死，但手术可引起患者产生应激性反应，术后并发症较多，患者可因此产生紧张、焦虑、抑郁、恐惧等负面情绪，影响术后恢复[2]。故在围手术期实施有效的护理以缓解股骨头坏死患者负面情绪，调节心理健康状态，对改善肢体功能具有重要意义[3]。音乐疗法是集音乐、心理、医疗为一体的交叉边缘性方法，能够对神经系统、下丘脑-垂体-肾上腺轴、心血管等系统产生积极的刺激作用，進而调节患者心理及生理状态。将音乐疗法与穴位按摩相结合进行护理，已在冠心病、妇科手术等疾病领域获得良好效果[4-5]。本研究对笔者所在医院52例股骨头坏死患者施行穴位按摩联合音乐疗法护理，取得较好临床效果，现报道如下。, http://www.100md.com(周丽华)
