无创机械通气治疗Ⅱ型呼吸衰竭患者应用预见性护理的效果及对并发症发生率的影响(1)
【摘要】 目的:探討对无创机械通气治疗的Ⅱ型呼吸衰竭患者实施预见性护理的临床效果。方法:选取2018年1月-2019年5月在笔者所在医院行无创机械通气治疗的Ⅱ型呼吸衰竭患者112例,按照随机数字表法将其分为观察组和对照组,各56例。患者均根据病情给予对症治疗、无创机械通气辅助治疗。在此基础上对照组实施常规护理,观察组实施预见性护理。比较两组日排痰量、机械通气时间、顺利完成治疗率、治疗前后血气分析指标、并发症发生情况。结果:观察组日排痰量、顺利完成治疗率均优于对照组,机械通气时间短于对照组,差异均有统计学意义(P<0.05)。观察组治疗后2、24 h氧分压、血氧饱和度均优于对照组,差异有统计学意义(P<0.05)。观察组胃肠胀气、面部压伤发生率及并发症总发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论:对无创机械通气治疗的Ⅱ型呼吸衰竭患者实施预见性护理,能显著提高临床效果,降低并发症发生率。

[Abstract] Objective: To explore the clinical effect of predictive nursing on the patients with type Ⅱ respiratory failure treated by non-invasive mechanical ventilation. Method: A total of 112 patients with type Ⅱ respiratory failure who were treated by non-invasive mechanical ventilation in our hospital from January 2018 to May 2019 were selected. According to the random number table, they were divided into the observation group and the control group, with 56 cases in each group. All patients were given symptomatic treatment and non-invasive mechanical ventilation assistant treatment according to their condition. On this basis, the control group was given routine nursing, while the observation group was given predictive nursing. The daily sputum excretion, mechanical ventilation time, successful completion rate of treatment, blood gas analysis index before and after treatment, and occurrence of complications were compared between the two groups. Result: The daily sputum excretion and successful completion rate of treatment in the observation group were better than those in the control group, and the mechanical ventilation time was shorter than that in the control group, the differences were statistically significant (P<0.05). The oxygen partial pressure and blood oxygen saturation of the observation group at 2 and 24 h after treatment were better than those of the control group, the differences were statistically significant (P<0.05). The incidence of gastrointestinal flatulence, facial crush injury and the total incidence of complications in the observation group were significantly lower than those in the control group, the differences were statistically significant (P<0.05).
