甲状腺大部切除术切口美容缝合及负压引流的临床应用(1)
【摘要】 目的：探讨甲状腺大部切除术切口皮内美容缝合及负压引流对切口愈合的应用效果。方法：选取2019年8-12月到笔者所在医院行甲状腺大部切除术的患者15例，常规沿皮纹作横形切口行甲状腺大部切除术，将切口皮内美容缝合，另外将切口置管于胸骨切迹上缘戳口引出接负压瓶行负压引流，对患者临床应用效果进行评价。结果：15例患者切口平均长度(3.4±0.3)cm，平均手术时间(50.0±5.5)min，平均住院时间(4.0±0.5)d，术中平均出血量(12.0±2.5)ml;术后切口甲级愈合率93.3%，切口无菌性炎症改变率6.7%，患者治疗总满意度100%;患者切口缝合瘢痕小，无积液。结论：甲状腺大部切除术切口采用皮内美容缝合加负压引流，具有切口小、恢复快、出血量少、美容效果好等优点，能够提高患者满意度，值得基层医院推广应用。

[Abstract] Objective： To investigate the effect of intracutaneous cosmetic suturing and negative pressure drainage on wound healing in subtotal thyroidectomy. Method： Fifteen patients who received subtotal thyroidectomy in our hospital from August to December 2019 were selected as research materials. A transverse thyroid incision along the skin line was used for subtotal thyroidectomy， and the incision was sutured intracutaneously. A negative pressure bottle was drained from the puncture at the upper edge of the sternal notch to perform a negative pressure drainage， and the clinical application effect of the patient was retrospectively evaluated. Result： The average length of the incision in 15 patients was (3.4±0.3) cm， the average operation time was (50.0±5.5) min， the average hospital stay was (4.0±0.5) d， and the average blood loss during the operation was (12.0±2.5) ml. The postoperative incision grade A healing rate was 93.3%， the rate of aseptic inflammation change in the incision was 6.7%， and the patients’ satisfaction was 100%. The scar of incision suture was small， and had no effusion. Conclusion： The incision of subtotal thyroidectomy adopts intradermal cosmetic suture and negative pressure drainage， which has the advantages of small incision， quick recovery， less bleeding， good cosmetic effect， etc.， and can improve patient satisfaction， which is worthy of promotion and application in primary hospitals.
