全髋关节翻修术中髋臼骨缺损的处理及其疗效分析任燕春(2)
本组22例患者主要为髋臼节段性兼腔隙性缺损,属于AAOSⅢ型,即混合型缺损,严重缺损的采用紧压式颗粒性植骨的方式进行植骨重建,加用生物型髋臼或髋臼加强环罩。紧压式颗粒性植骨主要适应于直径<10mm的缺损,其优点主要体现在:a)有利于释放生长因子,发挥骨诱导作用,快速再血管化和再塑形;b)宿主骨易向移植骨爬行替代;c)移植骨的顺应性或弹性在载荷作用下可产生形变,刺激骨生长,强度随着时间的推移逐渐加强;d)形成紧密接触的内衬,假体髋臼应力均匀、稳定[10]。本组病例手术严格遵从此方法。
总之,全髋关节翻修术中髋臼重建是一个具有挑战性的工作,术中彻底清除疤痕及肉芽组织,建立良好的骨床,骨床重建以后,人工髋臼要根据骨床的承受力来选择和固定,尤其是髋臼外上方,这是翻修术成功的关键。术后指导性锻炼与活动等,也是翻修术髋臼骨缺损植骨和假体植入成功的重要因素。目前生物型髋臼、特殊的髋臼假体,或加自体、异体颗粒性骨移植的植入都取得了较好的短期疗效,长期疗效尚待进一步观察。

总之,全髋关节翻修术中髋臼重建是一个具有挑战性的工作,术中彻底清除疤痕及肉芽组织,建立良好的骨床,骨床重建以后,人工髋臼要根据骨床的承受力来选择和固定,尤其是髋臼外上方,这是翻修术成功的关键。术后指导性锻炼与活动等,也是翻修术髋臼骨缺损植骨和假体植入成功的重要因素。目前生物型髋臼、特殊的髋臼假体,或加自体、异体颗粒性骨移植的植入都取得了较好的短期疗效,长期疗效尚待进一步观察。
