肿瘤标志物在肺癌患者中的表达及其临床意义(1)

【摘要】目的：探讨血清肿瘤标志物CA50、CEA、Cyfra21-1和Scc在肺癌诊断中的价值。方法：采用化学发光法检测140例肺癌患者、60例肺良性病变患者及80例健康体检者血清CA50、CEA、Cyfra21-1和Scc的含量，并进行统计学分析。结果：肺癌患者CA50、CEA、Cyfra21-1和Scc的阳性率分别为45.7%、65.7%、58.5%和59.3%，显著高于肺部良性病变患者和健康对照组(P﹤0.05)。CA50、CEA、Cyfra21-1和Scc小细胞肺癌患者较非小细胞肺癌患者表达水平低。CA50和CEA在肺癌高表达；而Cyfhl2I-l在肺鳞癌高表达。结论：血清CA50、CEA、Cyfra21-1和Scc检测对于肺癌诊断敏感性和特异性强，联合检测有助于对肺癌的诊治以及病情的判断分析。

The expression of tumor markers in lung cancer patients and its clinical significance

【Abstract】Objective: Explore the value of serum tumor markers CA50, CEA, Cyfra21-1 and Scc in the diagnosis of lung cancer. Method: Use chemical luminescence method to detect the serum levels of CA50、CEA、Cyfra21-1 and Scc in 140 patients with lung cancer and 60 Patients with benign pulmonary disease and 80 healthy controlss, and statistical analysis. Results: CA50, CEA, Cyfra21-1 and Scc positive rate in lung cancer patients were 45.7%, 65.7%, 58.5% and 59.3%, Significantly higher than those of benign lung disease patients and healthy controls(P﹤0.05). CA50, CEA, Cyfra21-1 and Scc expression in small cell lung cancer patients is less than those in non-small cell lung cancer patients. CA50 and CEA in lung cancer high expression; And Cyfhl2I - l in lung squamous cell carcinomas high expression. Conclusion: the Serum CA50, CEA, Cyfra21-1, and Scc detection is high sensitivity and specificity for diagnosis of lung cancer, Joint detection is help to the diagnosis and treatment of lung cancer, and judgments of the disease.

肿瘤标志物是细胞癌变过程中所产生的正常细胞缺乏或含量极微的特异性和相对特异性的物质，也可能是宿主细胞针对癌细胞所产生的正常细胞成分，但在量和质上与正常状态或良性疾病时明显不同。这些物质不存在于正常成人组织而仅见于胚胎组织，或在肿瘤组织中含量超过正常含量。血清肿瘤标志物包括癌胚抗原类、糖蛋白和蛋白类、酶类和激素类。肿瘤标志物在肿瘤诊断，肿瘤复发与转移监测，疗效和预后判断等方面都有较大的实用价值。肺癌是我国常见的恶性肿瘤一，发病率和死亡率逐年上升。近年来，随着肿瘤标志物的研究和应用，为肺癌的早期诊断、疗效监测和预后评估提供了便捷而又无创伤的诊断方法。本文对140例肺癌患者血清肿瘤标志物糖类抗原A50(CA50)、癌胚抗原(CEA)、细胞角蛋白19片断21-1(Cyfra21-1)和鳞状细胞相关抗原(Scc)的检测结果进行了分析，现报道如下。
